Dublin): what Tusla inspections found — 2 published inspection(s), non-compliances and the provider's corrective actions. ## The Children's Place Full Day · 1 - 6 Years · Dublin 1, Dublin · Tusla ID **TU2015DY210** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 25 February 2025 — Inspection Report Full report (PDF, Tusla) ##### Regulation 21 — Equipment and materials - The registered provider did not ensure the play and work materials in the Wobbler room adequately met the needs of the children in that room: 1. The play kitchen in the room was not a suitable height for the children in the room. It was too high for the children to adequately engage in successful play, for example the microwave press section was out of reach of the children. 2. The materials in the room were not displayed or organised in manner to encourage children to meaningfully engage in successful play experiences: o The materials were not stored in a grouped or themed manner; for example, a play box contained unrelated items such as rattles, cars, plastic fruit and blocks. o The props for the kitchen were not stored adjacent to the kitchen - 1. The kitchen was replaced with a smaller unit more suited to the children in the room. 2. The room has been divided into specific areas, and the service will ensure to keep these clear divisions in place ##### Regulation 22 — Food and drink - The provision of alternative meals was not in line with national guidelines on food and nutrition which state that a healthy alternative should be offered. A child was offered a brioche as an alternative in the Wobbler room, and staff in the pre-Montessori reported that children are offered a brioche or breadsticks if they do not eat their hot meal. Every hot meal should include ½ to 1 serving of vegetables, 1 serving of either bread, potatoes, pasta and rice, and 1 serving of either meat, poultry, fish, eggs, beans and cheese - The service had a meeting with staff and ensure brioche of breadsticks will not be given as an alternative meal and outlined that appropriate healthy alternative meals are available ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. A cot was positioned near to a cordoned off radiator where the temperature of the radiator was recorded as 50.1oC by the inspector. There was a potential risk the sleeping child in the cot could have access to the radiator. Infection Control: The following increased the potential risk of infection: 2. Children’s hands were not washed in the Wobbler room prior to dinner. 3. Not all of the cots had a removeable mattress covers in place, and the mattresses in these cots did not have a wipeable surface. 4. Two cots in the cot room were positioned directly adjacent to each other whilst children were sleeping. Cots are required to be positioned a minimum of 50cm apart whilst children are sleeping. Action submitted by the Registered Provider - General Safety: 1. The sleep room has been rearranged, with cots positioned away from the radiator. This has been marked out on the floor with tape. Infection Control: 2. Staff were reminded of the importance of handwashing, and a checklist was introduced for handwashing. 3. The service purchased additional mattress protectors to ensure spares are always available. 4. The sleep room has been rearranged, with cots positioned 50cms apart. This has been marked out on the floor with tape Found compliant: Regulation 9, 11, 19, 28. #### Inspection of 17 January 2024 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** An immediate action notice was issued to the registered provider on the day of inspection in relation to a non- compliance identified under ##### Regulation 9 — Management and recruitment - (2)(a)(b) Written and validated references were not available for three adults. This was a non-compliance on the previous inspection in March 2023. The preventive actions proposed by the registered provider have not prevented recurrence. (c) A Garda vetting disclosure was not available for review in relation to one staff member who had commenced working with the children. An immediate action notice was issued to the registered provider. (d) Evidence was not available to show that one staff member did not require Police vetting. (4) Evidence was not available to show that one staff member who worked directly with children attending the service held at least a major award in Early Childhood Care and Education at Level 5 or above on the National Framework of Qualifications or a qualification deemed eligible by the Department of Children and Youth Affairs. This was a non-compliance on the previous inspection in March 2023. The preventive actions proposed by the registered provider have not prevented recurrence - (2)(a)(b) Validated references are now attached to the staff members files. The registered provider will ensure that all references will be verified including students. The service will be more mindful in future to ensure to attach all paperwork. (c) The staff member did not attend the service following the inspection until Garda vetting was received. The registered provider will ensure that this is checked in future. (d) The registered provider will ensure that anyone who has lived outside Ireland will hold police vetting. (4) The staff member has applied to have the qualification approved with the DCEDIY. In future the service will send the qualifications to the DCEDIY for approval ##### Regulation 19 — Health, welfare and development of child - 1. The sleep needs of one child in the Wobbler room were observed to not be met; The child appeared tired and was observed crying between 11:40am when the children started being put into the cots for naptime until 12:38pm. Staff advised that the child has two naps per day and would be put to bed at 2pm. Staff reported the child had had a significantly shorter than normal morning nap in the morning and staff were heard to comment that the child was tired however they did not place the child down for a nap despite cots being available for use. Sleep is a vital for learning and development of children and a needs-based approach is required. 2. Staff did not respond adequately to the care needs of one child aged 11 months between 10:05-11:45am. The top being worn by the child was observed to be wet from the collar down to the waistband of the child’s trousers at 10:05am. Staff were observed to engage the child in a number of care practices including holding, feeding and nappy changing and acknowledged the child’s top was wet from dribbling due to teething however the child’s top remained unchanged until 11:45am following a prompt from another adult. Wet clothes should be changed to ensure the child’s comfort. 3. Materials in the Wobbler room were not available or were displayed in such a way as to limit opportunities for spontaneous engagement for example supporting equipment was not readily available for use with the play kitchen and books were stored face down on a shelf in the centre of the play kitchen which prevented children seeing what books were available to choose. Grouping materials in a thematic way, readily available and displayed appropriately supports children to engage in play experiences in a spontaneous way - 1. The registered provider spoke to the parents and discussed this with them and they now understand it is not always possible to follow the 9am and 2pm schedule. 2. The registered provider spoke to the staff about the importance of changing children’s clothes if they are damp. Staff are aware of the importance of checking the children’s clothes at all times. 3. Staff have rearranged the toys and library books, so they are more accessible at all times ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. Radiator temperatures were not thermostatically controlled and were hot to touch, posing a risk of scalding. The following temperatures were recorded; • In the Wobbler room the radiator was 52.8℃ at 10:55am • Hall adjacent to the Wobbler/Toddler room nappy changing bays 55.1℃ at 11:45am • In the Toddler room 55.7℃ at 12pm An immediate action notice was issued in relation to above due to the increased risk of scalding. Radiators with a surface temperature of over 50℃ require a cover. 2. Water was not thermostatically controlled at the low sink beside the nappy changing unit in the Sanitary facilities at mezzanine level. The water temperature was recorded as 46.6℃ at 12:40pm. 3. A low level, coloured fence which was stored adjacent to the outdoor toilet was observed to be broken with sharp edges and screws exposed at a level which could cause injury to the children. 4. Cleaning spray was stored at a level accessible to the children in the Pre-montessori room. This presents a potential risk of injury to the children should they access the spray. Infection Control: 5. The cover on the sandpit was not effectively secured at the front which left some of the sandpit uncovered and accessible to animals. Sandpits must be covered for infection control purposes. 6. Four sinks in the sanitary area at mezzanine level used by the Pre-montessori and Montessori classrooms did not have an adequate water supply for example no water or a trickle of water came out of the taps when turned on. An adequate flow of warm water is required for effective handwashing to reduce the spread of infection. 7. Low level beds used by the children in the Toddler room and Pre-montessori room were observed to not be placed 50cms apart and in some cases were touching. This posed an infection control risk. 8. The nappy changing mat on one unit in the Toddler room nappy changing bay was torn with the foam exposed. This posed an infection control risk as the mat could not be effectively cleaned. Administration of Medication: 9. Staff in the Montessori room were unclear of the current health care plan for a child who had previously required emergency medication. Staff advised the child no longer needed the medication however there was no paperwork from the parents advising of this. Health care plans should be updated regularly to ensure children receive appropriate care in the event of an emergency. 10. Emergency medication which was available for a child in the montessori room was out of date and had not been disposed of in line with the service policy. Action submitted by the Registered Provider - General Safety: 1. The plumber adjusted all the thermostats. The plumber adjusted all the thermostats on the radiators, and the service is in the process of putting guards around the radiators. 2. The plumber adjusted the thermostat. The plumber has changed the water feed so that this is on a separate thermostat. 3. The fence has been removed. 4. The importance of replacing the cleaning sprays on the shelf provided after use was discussed with the staff members. Infection Control: 5. The service will have the existing cover replaced with a roll out cover with clips both ends. 6. The plumber was there on the day of the inspection, the registered provider had called him that morning as the water pressure was so low. He replaced the existing water pump with a new one. This was fixed on the evening of the inspection. 7. The beds have been moved around to keep them apart. 8. This was replaced immediately. The registered provider spoke to the staff about this and ensured everyone knows where the new ones are stored and how important it is to replace them immediately. Administration of Medication: 9. The registered provider spoke to the staff about correct handover of care plans. 10. The registered provider discussed the importance of checking best before dates even when the service receive the medication from the parent ##### Regulation 28 — Insurance - The registered provider did not ensure the service was adequately insured for the number of children in attendance for example 84 children were in attendance at 2:45pm. The insurance in place covered 80 children at any one time.
