**Immediate action notice.** This inspection was unannounced and focused on the area of governance. The inspection may also focus on other areas as required. The inspection focused on an examination of compliance under regulations 9,11 and 15 on inspection additional non-compliance which posed a risk was identified under Regulation 8. These findings are outlined within the relevant regulation within this report.

##### Regulation not named in the report text

- (1)The registered provider upgraded and changed the prefab that was previously used in March 2023 and this change to the service was not notified to the National Registration Office prior to opening

- The registered provider stated the following in their reply. Corrective and Preventive Action (1) An application for change of circumstance ,will be submitted to the National Registration Office. Any future changes to the building will not proceed without giving Tulsa a minimum of 60 days’ notice in writing of any proposed changes. Summary Comment The regulatory requirement of this regulation remains non-compliant, as the application for change in circumstances is still outstanding
