General Safety: 1. The inspectors observed that two televisions positioned on a low- level shelving unit in the Tweenies room were not securely anchored to the surface. This posed a risk of the televisions tipping over and increases the risk of injury to a child. 2. Three cables in the Bright Stars room were not safely secured to the wall. A cable from an air conditioning unit, a clock and a thermometer were not safely secured and out of reach of children, posing a potential risk to their safety. Infection Control: 3. Appropriate handwashing practices were not in place to prevent the spread of infection as follows: • A staff member was observed changing several children’s nappies without washing either their own hands or the children’s hands after each change. Additionally, the staff member used their hand to open the lid of the pedal bin when disposing of waste. • A staff member did not wash their hands immediately after changing a child’s nappy. Prior to washing their hands, the adult was observed to take another child by the hand and lead them to the nappy room at which time they washed their hands. Additionally, the staff member used their hand to open the pedal operated bin and dispose of the waste. The above practices pose a risk of the spread of infection and illness to a child. Action submitted by the Registered Provider

Provider's corrective action:

General Safety: 1. The registered provider stated that the tv has been removed and placed in the storeroom and that normally there are safety fences along that side of the room preventing children to gain access to the area. However, on the day of the inspection a staff member removed the fencing. Staff have been informed that if the tv is in use for the afterschool children in the afternoons, it must be returned to the storeroom. 2. The air purifier and cables have been removed from the care room. Staff have been made aware of the dangers of trailing cables and will ensure no cables are unsafe going forward. Infection Control: 3. The staff member has been retrained in the nappy changing policy and has assured management that they are aware of the procedure. Management go through the policies with each staff member on a weekly basis, to ensure that all staff are aware of all procedures within the creche

(d) Police vetting was not available in relation to three adults who previously lived outside of the state for a period of more than 6 months as an adult. (3) Documentation reviewed evidence that the procedures specified above under 9(2)(d) had not been carried out prior to the three adults commencing employment in the service

Provider's corrective action:

2 (d) Police vetting has been obtained for one staff member and applications have been submitted for the remaining two staff members. A comprehensive checklist has been developed outlining all required documents ensuring consistency and clarity for both management and new hires

General Safety: 1. During the inspection, it was observed that babies bottles were not stored in the fridge, which was at variance with the services healthy eating policy and poses a risk of bacterial growth potentially leading to illness in a baby. The inspectors observed that bottles were stored on a shelf in the baby room and conversation with staff confirmed that this is part of their daily practice. Infection Control: 2. Two cots with side bars situated in the sleep room were positioned directly beside each other posing a risk of cross contamination. Some of the nappy changing practices observed posed a risk of cross contamination and were at variance with the services nappy changing policy: - A staff member was observed cleaning the soiled changing mat after washing their hands following nappy changing. - It was observed that gloves worn during nappy changes were not removed before placing clean nappies on the babies. - Babies hands were not washed effectively following nappy changing. While it was observed that wipes were used, this method is not considered effective in the removal of germs. . Safe Sleep: 3. Practice observed in the Baby room did not align with safe sleep guidance. During the inspection it was observed that an infant was sleeping in a rocker in the care room. This practice poses a risk to the infants safety. Action submitted by the Registered Provider

General Safety: 1. Babies bottles are now stored in the fridge and removed prior to feeding. Infection Control:
-A smaller cot has been purchased to allow for more distance between the two cots. -All staff have been retrained in the nappy changing procedure. Safe Sleep: 3. Staff have been informed that going forward if a child falls asleep, they must be transferred to a cot
