##### Regulation 9 — Management and recruitment

- (2) (a)(b) One written and validated reference was not available for one adult. (b) One written reference for one adult from a source other than a past employer was not validated

- (2)(a)(b) One staff member no longer works in the service and the reference from a source other than a past employer was validated. The registered provider will ensure that all references are obtained and validated prior to staff or students starting in the service as part of the service Recruitment Process

##### Regulation 26 — Fire safety measures

- (b) A record was not available to demonstrate annual service of the smoke alarm was competed with the number, type and maintenance for the smoke alarms in the premises. It is acknowledged that evidence of maintenance completed on the 25 June 2026, was submitted by the registered provider post inspection

- (b) The maintenance of smoke alarms was completed 25 June 2026 and the record was made available and submitted to the agency 25 June 2026. The service has renewed their contract, and maintenance will be completed annually
