Dublin): what Tusla inspections found — 3 published inspection(s), non-compliances and the provider's corrective actions. ## Saint Ultans Childcare Project Sessional · 0 - 6 Years · Dublin 10, Dublin · Tusla ID **TU2015DY378** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 19 June 2025 — Inspection Report Full report (PDF, Tusla) ##### Regulation 19 — Health, welfare and development of child - In the Palm room the transition from lunch to sleep time was observed to be unsettled and disorganised. 1. At approximately 12:33 two children were placed into their cots to rest by staff. The children were left unattended to self-settle. During this time, both children were observed jumping in their cots. Although staff entered the cot room briefly, they did not remain present with the children during the settling period. 2. Another child displaying signs of tiredness, at 12:30 did not have his needs met in a timely manner, as he was not offered an opportunity to sleep. Upon discussion, staff indicated they intended to wait until two other children had settled before placing him to rest. The child was eventually placed at 13:14 to sleep following prompting by the inspector - 1. The registered provider has stated an additional member of staff has been scheduled to support staff in the palm room during transitions. The staff member will also stay with the children in the cot room to help them settle to sleep. A staff meeting was held where all staff had the opportunity to discuss transitions. 2. The registered provider has stated that through discussion with staff, a plan has been developed to ensure all children’s needs are met, the additional staff member scheduled to support the room will ensure this plan is effective ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. In the palm room, the appropriate room temperature of 18-22 degrees was not maintained throughout the inspection, the temperature reached 25.1 degrees. This poses a risk of overheating to children. Infection Control: 2. In the sanitary area in the willow room the nappy changing mat was observed to be torn with foam exposed which prevented adequate cleaning and posed an infection control risk. 3. In the sanitary area in willow room, the lacquered surface around the nappy changing mat was observed to be worn and could not be cleaned effectively. 4. In the chestnut room, a cloth towel was available in the sanitary area. Staff reported that some children use this communal cloth towel when drying their hands, this poses a potential risk of cross infection. Action submitted by the Registered Provider - General Safety: 1. The registered provider has stated that staff will actively try to reduce the temperature in the rooms by opening windows and pulling blinds down when necessary. A quote for an air conditioning system has also been obtained. The service has applied to a funding body for support to purchase the air conditioning system. Infection Control: 2. A new nappy changing mat has been purchased and is in place in the nappy changing area in the willow room. 3. The Lacquered surface around the changing mat has been repaired and repainted. 4. The cloth towel has been removed from the sanitary area in the chestnut room, paper towel will be used at all times Found compliant: Regulation 9, 11, 28. #### Inspection of 12 June 2024 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** On 12 June 2024, an Immediate Action Notice was issued to the registered provider. Concerns were identified as an immediate and significant risk or is likely to be a risk to the safety of the children attending the service which is contrary to the requirement of ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. On 12 June 2024, an Immediate Action Notice was issued to the registered provider as 16 blind cords were not secured to the wall or broken. Nine of these were in reach of the children and posed a risk of strangulation. The registered provider submitted an adequate response on 13 June 2024 to the concerns identified which addressed the risk. 2. Garda vetting was available for one adult, however, this vetting disclosure was not dated within the previous three years in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting. Infection Control: Infection control practices observed during the inspection were ineffective and posed a risk of cross infection and were at variance with the service’s policy as outlined below: 1. During the nappy changing practices, staff were observed to redress children with the same used gloves throughout the procedure. 2. The nappy changing mat used by the children from the Olive room could not be cleaned effectively. Inspectors observed a large tear in the mat with the inner foam exposed. 3. Three open top bins were observed in the Cabin, Maple and Oak rooms. 4. Adults and children were observed lifting the lid of the bins with their hands and not using the foot pedal provided. Administration of Medication: A sample of medication records were reviewed. Four of these records were not completed as per the policy of the service. 1. Two records of medication which was given, had the child’s first name only. No parental consent for giving this medication was recorded. 2. Two records of medication did not have any identifying details for the children to whom the medication was given. There was no parental consent recorded to give this medication. Action submitted by the Registered Provider - General Safety: 1. On 13 June 2024, the service secured and repaired all 16 blind cords. The service has started the process of replacing any old blinds and will have any broken/worn blinds replaced over the summer weeks. The manager has spoken to staff at a staff meeting and asked all staff to let the manager know if any blind cords are not secured so we can immediately have the cords secured to the wall and completely out of the reach of children. 2. Renewal of Garda vetting was sent off on the day of inspection and has since returned complete. Ensure Garda vetting is dated within the previous three years by checking staff files on a regular basis. Infection Control: 1. Nappy changing policy was given to staff to re-read and when the nappy changing procedure was understood we asked all staff members to sign the document. Nappy changing procedure is now printed and on display in every nappy changing facility in the building. 2. Nappy Changing mat was removed immediately and replaced with new nappy changing mat. Ensure nappy changing mats are checked on a regular basis and we asked staff to please inform us if any equipment is in any way damaged so it can be discarded of immediately. 3. Open top bins were immediately removed from the three rooms and replaced with closed lid pedal bins. Ensure all bins are checked on a regular basis to guarantee all bins are closed lid bins with the foot pedal in working order. 4. The manager to staff in our staff meeting to please ensure that the pedals are used on the bins at all times and to teach the children to use the foot pedals at all times. Infection control policy was given to all staff to re-read and sign. Administration of Medication: 1. The manager has spoken to the individual staff members responsible for the incomplete forms and explained the importance of ensuring that forms are completed in full. 2. The manager has asked all staff members to read and sign a document which gives a in detail description of the procedure of administrating medication within the service Found compliant: Regulation 9, 11, 15, 16, 25, 26. #### Inspection of 25 September 2023 — Inspection Report Full report (PDF, Tusla) ##### Regulation 9 — Management and recruitment - (2) The registered provider did not take appropriate measures to ensure adults were suitable and competent to work in an early years service as outlined below: (a) & (b) A reference from a source other than a past employer was available but was not validated in respect of one adult. Four validated references from either a past employer or a source other than a past employer were not available in respect of three adults. (4) There was no evidence that five adults working directly with the children had attained at least a major award in Early Childhood Care and Education at Level 5 on the National Framework for Qualifications or a qualification deemed to be equivalent - (a) and (b) References were validated and appropriate references were sourced. (4) Outstanding qualifications for staff submitted ##### Regulation 23 — Safeguarding health, safety and welfare of child - The registered provider did not notify the Agency in writing of the introduction of a new structure to be used as a care room. On the day of inspection, the service was providing a sessional service for 8 preschool children in a newly built structure called The Cabin. This structure had not been inspected to confirm if it was fit for purpose by the inspectorate - Change of Circumstance form submitted. Additional information required by the registration office. Awaiting fire certificate. In the meantime, the cabin is not in use. This does not affect the operation of the service ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: On the day of inspection, two children were transitioning between rooms for different periods. The children were not recorded on the attendance record in the room they moved to, only in their initial room. Lack of clarity about the location of children within the service presents a risk in the event of emergency evacuation. Action submitted by the Registered Provider - General Safety: Room transfer forms are in place.
