(1) A sample of twelve children’s registration records were reviewed as part of the inspection. The registered provider ensured a full record (1) (a-f) and (h-i). Registration records did not include (g) the name and telephone number of the child’s registered medical practitioner. It is important to have this included so that swift and prompt medical treatment can be administered to children in an emergency

(1) The service has added (g) the name and the telephone number of the child’s medical registered medical practitioner, to the registration forms and requested families to forward on the information. Registration forms are now updated to ensure all the correct information is collected before a child starts in our service
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: 1. A safety lock on a press containing hazardous cleaning materials was broken and as a result hazardous cleaning products were potentially accessible to children. It is acknowledged that cleaning products were removed during inspection and a new safety lock was installed by 3pm on the day of inspection. 2. A storage cupboard on the first floor adjacent to the preschool room which stored hazardous cleaning materials was unlocked. It is acknowledged that during the inspection this was rectified. 3. The sanitary basin where all children in the service washed their hands after outdoor play did not have warm running water. The water temperature was recorded at 10°C which does not allow for effective hand washing in line with HSPC guidance in relation to infection control protection measures and the services hand washing policy which states that “children should rinse their hands under running water, no less than 15°C and no more than 43°C until they are free of soap and dirt”. 4. Staff belongings including handbags were accessible to children and posed a potential safety risk to children. Infection Control: 5. Nappy changing and hand washing was observed to be at variance with the service nappy changing and hand- washing policies and was not in line with best practice. This was evidenced by the following: o Disposable gloves were not used effectively to eliminate the risk of cross contamination during nappy changing. o Six children were observed not to have their hands washed after nappy changing. o Staff members did not was their own hands after changing children’s nappies. o The nappy changing unit was not effectively cleaned during nappy changes. This posed a risk of infection and cross contamination. 6. The covering on the sofas in Wobbler room and in the outdoor play shop were ripped with foam exposed which does not allowed for adequate cleaning and posed an infection risk. General Safety: 1. This was rectified on the day of inspection; all products were removed and a new lock purchased and installed during the inspection. A sign has been attached to the door to remind staff to keep it locked at all times. 2. The service attached a sign to the door to remind staff to keep it locked at all times and have permanently removed the cleaning products from this press. This was actioned on the day of inspection. 3. Staff have been advised to no longer use that sink as it can’t be thermostatically controlled. The service has reverted to pre covid routine of washing the children’s hands after garden time upstairs in the children’s sanitary area where there is sufficient warm running water and two sinks. 4. Staff belongings were removed and placed on the top shelf out of reach from children and staff have been advised to keep their belongings on the high shelf of the storage unit out of reach from children. Infection Control: 5. Initially, relevant staff were issued with a copy of the policy and procedure and advised completing the nappy changing procedure training again. Relevant staff members have now completed nappy changing training as there was a failure to follow the correct procedure and hygiene standards risking contamination and infection. 6. The sofa in the wobbler room has been reupholstered, the one in the garden has been discarded. Staff have been reminded to ensure they inform management of any materials or furniture that needs replacing or repair

Immediate action notice. An immediate action notice was issued to the registered provider on 30 July 2025 in relation to a concern identified under Regulation 23, Safeguarding the Health, Welfare and Development of child. A response which adequately mitigated these concerns was received on 31 July 2025. Further details are available under Regulation 23.
Regulation 9 — Management and recruitment
(7) (c) Staff supervision The registered provider did not ensure that staff had been provided with appropriate ongoing supervision which can have result in potential gaps in safe practice. This is evidenced in the non-compliance under Regulation 16. There was documentary evidence to show that the roll out of staff supervision, staff appraisals and annual reviews were at variance with the service’s policy. Out of seven staff supervision records, o There was no documented evidence available that there was ongoing support and supervision for three staff members. o Two out of seven supervision records did not include a staff or management signature. o Two out of seven records did not have the date that the supervision took place. o Documentary evidence available showed one staff member had not received support or supervision since 2022, and another had not received support or supervision since 2017 Staff require documented ongoing support and supervision in order to effectively meet the requirements of the regulations
Provider's corrective action:
Supervision records have been updated for staff members. More care will be taken to ensure staff supervision meetings are more frequent and filed accordingly
Regulation 16 — Record in relation to pre-school service
(1) (k) A review of documentation and discussion with staff identified that a recent incident that had occurred in the service which was not reported to parents. Parents must be informed of any incidents involving their child in order to be informed of the steps that they may need to take to provide for their appropriate care. This was at variance with the services policy. It is acknowledged that completed accident and incident reports were maintained
Provider's corrective action:
Parents were called in for a meeting the next day and informed of the alleged incident involving their child. In future our policies will be carried out diligently regarding accidents and incidents
Regulation 19 — Health, welfare and development of child
(1) (b) There was no individual care plan available for one child who, through discussion with staff and documentary evidence showed signs requiring extra support around sensory-seeking behaviour. An individual care plan helps outline evidence of a child's strengths, challenges, or goals
Provider's corrective action:
We have reminded all staff to use their training regarding supporting positive behaviour. We have displays in all our classrooms regarding challenging behaviour. All staff will follow and implement our Supporting Positive Behaviour Policy
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: 1. Under the sink within the sanitary room used by pre-school children and toddlers was observed to be worn. When further inspected, and moved with ease, there was a nail protruding underneath that posed a potential puncture risk to a child. Safe Sleep: 2. The recommended ambient sleep temperature between 18-22℃ was not maintained in the wobbler sleep room where children over one years old were sleeping. At 13:10pm the room temperature was recorded at 23.8℃ on the inspector’s calibrated thermometer. Fire Safety: 3. The emergency fire exit door in the partitioned cot room within the wobbler room was observed to be obstructed with the positioning of one cot while 5 children were sleeping at 13:05pm. In addition, the other side of the sleep area which is a part of the fire escape route was obstructed by a wooden divider that partitions the room and an air-cooling system. This posed a risk of safety of children in the event of an emergency evacuation. An Immediate action notice was issued on the day of the inspection in relation to this concern. A response which addressed this non-compliance was received on 31 July 2025. Action submitted by the Registered Provider

General Safety: 1. The shelf has been repaired. Staff have been reminded to risk assess and tell management if repairs are required. Safe Sleep: 2. During the hot summer months, we will close the blinds earlier and add more ice to our air conditioning unit to try and cool the room. Fire Safety: 3. Actions were taken immediately on the day. Staff have signed a document to ensure the room remains laid out with a safe fire exit
(2)(a)(b) Evidence of a written validated reference was not available for one staff member
Provider's corrective action:
Corrective Action The service located the reference with evidence it was validated in 2019 when the employee started with Parkview. The reference had been misplaced in error. Preventive Action The service commits to being diligent with files and will continue to check files on a regular basis to ensure compliance at all times
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: 1. The corners of the two couches in the Wobbler room were torn, leaving the foam exposed. This posed a risk of choking if ingested by children. Infection Control: The following increased the potential risk of infection: 2. The radiator cover in the Wobbler room was broken. The paint on the radiator exposed was peeling leaving the surface un-wipeable and there was a build-up of dust and debris accessible to the children. 3. Three bottles containing milk were observed to be left stored on a shelf unrefrigerated from 9.44am when the inspector arrived into the Wobbler room until 11.02am when the inspector left the room. Fire Safety: 4. The details of the attendance of the children were not accurately recorded on the attendance book. Sixteen children were recorded as present when the inspector checked the attendance log at 10.02am and 10.40am when there were 18 children present. Conversation with staff established that this log is used to support fire evacuation drills. Contemporaneous accurate attendance logs must be maintained to support the safe evacuation of children in an emergency. Action submitted by the Registered Provider

General Safety: 1. Corrective action: a new couch was ordered for the room. Preventive action: the service commit to routine checks on the soft furnishings for rips or tears and replace immediately before they become a hazard. Staff were advised to report anything in their room’s that may need to be replaced. Infection Control: Corrective action 2. The radiator was replaced the next day. 3. The service reminded staff that bottles of milk are to be put directly into the fridge on arrival. Preventive action 2. The service commit to replace broken fixture and fittings in a timely manner, staff were advised to take note of any hazards in their room and to report it to management. 3. Management will include checks to ensure bottles have been put in the fridge as part of the daily checks. Fire Safety: 4. Corrective action: the service reminded staff of the importance of signing children in and out accurately. Preventive action: management will complete checks on the register to ensure accuracy
Regulation 25 — First aid
(1) Documentary evidence available demonstrated that staff members trained to First Aid Responder level were not available to the children attending the service at all times during the operational hours of the service. It is acknowledged that there was a staff member training in Paediatric First Aid available to the children

Corrective Action The roster was immediately reviewed to ensure cover for the opening hours of the service. Preventive Action The service ensure diligence to ensure they have the adequate FAR trained staff on site at all times
Regulation 27 — Supervision
On two occasions during the inspection children were observed to be left unsupervised. The following was observed: • Five children were left unsupervised in the Wobbler room when the staff member had left the room to answer the door to the inspectors. The door to the room was closed and the children were observed to be moving freely throughout the room with no supervision with one child observed to enter the cot room adjacent to this room. • Seven children were left unsupervised in the outdoor play area when the staff member went to check on a child who was using the downstairs toilet. It is acknowledged that the person in charge was available to provide cover on both of these occasions if requested
Provider's corrective action:
Corrective Action Staff were reminded to call for cover when needed at all times and not to leave children unsupervised for any reason. Preventive Action The manager will be available to staff to ensure they have everything they need, and for any cover if required
