Tipperary): what Tusla inspections found — 2 published inspection(s), non-compliances and the provider's corrective actions. ## Mullinahone Community Playgroup Sessional · 2 - 6 Years · Thurles, Tipperary · Tusla ID **TU2015TY134** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 6 October 2025 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** A referral to Services Operating Outside of Registration Status (SOORS) was made following the inspection due to a non-compliance under Regulation 8. ##### Regulation 15 — Record of pre-school child - A sample of 10 completed registration forms were reviewed as part of the inspection process. The service did not maintain a written record of the following required information on two children’s registration forms: (h) Record of immunisations. It is acknowledged that the remaining eight registration forms contained the required information as detailed under parts (a) to (i) of this regulation - The uncompleted registration forms have since been completed and relevant documentation provided. These records must be returned to the service prior to a child commencing in the service as stated in the admission policy. All forms will be checked by service manager on return ##### Regulation 23 — Safeguarding health, safety and welfare of child - Infection control: 1. Perishable foods were not stored in a fridge. This caused a risk of food spoilage and food borne illnesses. Action submitted by the Registered Provider - Infection control: Each morning the children’s perishable foods are placed in the fridge ##### Regulation 26 — Fire safety measures - (1)(a) There was no record of regular fire drills kept. The last recorded fire drill was in November 2024. It is acknowledged that staff reported monthly fire drills take place - Monthly fire drills have since been carried out in both morning and afternoon sessions. Fire drills are now scheduled monthly ##### Regulation not named in the report text - (1) The service was found not to have notified the registration department of the Early Years Inspectorate, in writing, of changes in the registered number of children the service could accommodate. The service was registered to provide care to 18 children. There were 19 children attending the morning session. A referral to Services Operating Outside of Registration Status (SOORS) was made following the inspection - Service has been contacted by the Regulatory Support Office. A change in circumstances is in the process of being submitted for the number of children attending the service Found compliant: Regulation 9, 11, 20, 25. #### Inspection of 14 September 2023 — Inspection Report Full report (PDF, Tusla) ##### Regulation 9 — Management and recruitment - (2)(a)(b) Two references available on file were not validated by the registered provider. (d) International police vetting was not available for one staff member who had lived outside the state for a period over 6 months. (3) The procedures specified in paragraph (2) were not completed in full for two staff members working in the service, who as part of their role was working directly with the children - (2)(a)(b) Two references available on file were validated by the named person in charge. (d) International police vetting has been submitted and awaiting confirmation for staff members. (3) All staff files have been checked to make sure files are validated references, Garda vetting and police vetting where applicable are held on file in respect of all staff working in the service. Staff files will be reviewed on an annual basis to ensure that the necessary documents as required are always held on file ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: The service did not have a means of storing perishable food such as yoghurts, cooked meats and cheeses between 0 to 5 degrees Celsius. Action submitted by the Registered Provider - General Safety: The service at the time of inspection did have a refrigerator for storing perishable foods, it was in the office next to the preschool room. The team will make sure to always store the perishable foods in this between 0 to 5 degrees for the foreseeable ##### Regulation 25 — First aid - (1) A person certified in a recognised PHECC First Aid Response (FAR) was not available on the premises on the day of inspection. It is acknowledged that a staff member was trained in basic first aid and available to children - A staff members PHECC First Aid Responder (FAR) has been traced and is now available on file. Also, we are going to make sure all staff members are training in First Aid Responders. We will make sure that there will always be a staff member available on the premises always trained as a First Aid Responder ##### Regulation not named in the report text - The registered provider was found not to have notified the registration department of the Early Years Inspectorate, in writing, of changes in the named person in charge.
