#### Inspection of 11 May 2026 — Inspection Report

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. The most recent Garda vetting disclosure presented in respect of one of the staff members was not dated within the previous three years in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI- RN12.3 Renewal of Garda Vetting’. Infection Control: 1. The nappy changing procedure was not carried out correctly in line with the service’s nappy changing procedure displayed which was inadequate for infection control purposes. • The child was redressed with the same gloves and apron worn for nappy changing. • The inspector had to prompt staff members to wash the child’s hands after nappy changing. Action submitted by the Registered Provider

- General Safety: The Garda Vetting disclosure for the staff member identified has now been renewed and an up-to-date Garda Vetting disclosure is on file. To prevent reoccurrence, the service has implemented a Garda Vetting monitoring system which records the date of each employee's vetting disclosure and renewal due date. The Manager will review the register regularly and initiate renewal applications well in advance of the three-year expiry period to ensure continuous compliance with Tusla Regulatory Notice. Infection Control: Following the inspection, a staff meeting was held on 21/5/2026 to review and retrain all staff on the service's Nappy Changing Procedure and Infection Control practices. To support consistent implementation, the nappy changing procedure has been updated and displayed in a clear visual picture format at all changing stations. Management will carry out ongoing observations and monitoring to ensure procedures are followed correctly and to maintain high standards of infection prevention and control

##### Regulation 9 — Management and recruitment

- Four references were not validated by the registered providers in respect of two adults working in the service

- The two outstanding references have been validated. The roles of general manager, service, manger, deputy manager, and supervisor, have been amended and clarified to separate the administrative tasks from supervisors’ tasks, and general budgets from service budgets in response to concerns raised by management that their supervisory and management roles were too broad, and contribute to confusion. The procedures of the Board of Directors has been amended to include the submission of a supervision report covering the supervision of any issues raised by Tusla for a period of two years. The report will be prepared in the creche by the Deputy Manager, who must physically check for continued compliance rather than verbally check

##### Regulation 11 — Staffing levels

- During the period 12:00 – 12:45 in the wobbler, junior toddler and senior toddler rooms the correct adult to child ratios were not maintained during nappy changing and when meals were being served and children were eating. A third qualified adult was required in each room to maintain the correct adult to child ratios when the second adult left the room to carry out nappy changing. • Seven children were left in the care of one adult in the wobbler room. • Ten children were left in the care of one adult in the junior toddler room. • Eleven children were left in the care of one adult in the senior toddler room

- A schedule for nappy changing was tested however it was not workable for the period from 12 to 2. Scheduled nappy changes covering the period from 10 to 11 were set, and staff are asked to anticipate when nappy changes are needed during this time and phone office ten minutes in advance. A record of the lunch nappy changing schedule and cover has been added in the office and will be monitored for June to ensure compliance. A notice has been placed above all the nappy changing mats to remind staff not to change nappies if it leaves the room out of ratio, and to ring the office if they need cover

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. The floor mat at the emergency exit door in the pre-school room did not have a non-slip backing which posed a potential slip hazard. Infection Control: 2. Seven highchairs were stored incorrectly in the sanitary accommodation next to the nappy changing bin which was ineffective for infection control purposes. 3. The five-point safety harness straps were heavily stained, discoloured and required cleaning or replacement. Action submitted by the Registered Provider

- General Safety: 1. The mat has been removed. Infection Control: 2 &3. Most of the highchairs have been thrown out, with small chairs brought in and 3 other chairs. We removed all the straps and washed them, and they came out very clean. By throwing out some of the chairs we have spares, and we have added changing the straps to the duties of the supervisor. Management have created the ancillary role of curriculum supervisor to ensure that the general supervisors role focuses more on cleanliness and QRF checks

##### Regulation 9 — Management and recruitment

- Two written validated references were not available from a previous employer or a source other than a past employer in respect of two staff members
- A completed Garda Vetting disclosure was not available in respect of one adult present in the service. An immediate action notice was issued to the registered providers in respect of Regulation 9 Management and Recruitment - (2) (c) Garda Vetting Disclosures

- Written references have been acquired for both staff members. (References included)
- Garda vetting has been acquired for the staff member. (Vetting included) As a preventative measure any new staff that joins the service will have their contract presented to them by the service provider as part of their induction procedure. This will not be possible without all references and vetting which places the service provider at the end of hiring new staff. The registered provider will ensure that all files are present before contracts are provided. Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate, reviewed by the Early Years Inspector and deemed to meet the regulatory requirement

- General Safety: 1. The floor mat at the home corner in the senior toddler room did not have a non-slip backing which posed a potential slip hazard. 2. The cords attached to the recently installed telephones in each of the pre-school rooms were low hanging and accessible to children which posed a potential risk of injury to a child. Action submitted by the Registered Provider

- General Safety: 1. A slip backing has been acquired and fitted on the floor mat in the senior toddler room. (Photograph included) 2. All telephone cords have been hung upwards out of reach of the children. (Photograph included) As a preventative measure the maintenance person is being trained to assess to look for environmental risks. This creates a triple lock system with the supervisor providing daily inspections, the maintenance person has been given the responsibility to highlight any risks, log and fix them. The service provider is doing full QRF checks during the three terms when the service is closed (Christmas, Easter, Summer). The service providers will also perform less detailed checks throughout the year. Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate, reviewed by the Early Years Inspector and deemed to meet the regulatory requirement

##### Regulation 29 — Premises

- (d) 1. The top bar of a cot in the baby sleep room easily came apart from the side bar and required securing into place. 2. The conduits that secured cables in the wobbler sleep room were not fully encased and required repair. 3. In the nappy changing area attached to the junior toddler room the sliding bolt on a cupboard door within was not functioning and required repair. 4. The ventilation grids in the sanitary accommodation attached to the two toddler rooms and the pre- school room were dusty with a build up of physical debris which required cleaning. 5. The two rest chairs in the pre-school room were heavily stained and required recovering or replacement. (e) Thermostatically controlled hot water was not available in the sanitary accommodation in the pre-school and midi rooms

- (d) 1. The top bar of the cot has been glued back in place. All other cots were thoroughly inspected for similar faults. (Photograph included) 2. The conduits have been replaced. (Photograph included) 3. The receiver for the sliding bolt has been replaced and the locking mechanism is fully functioning. (Photograph included) 4. The ventilation grids have been cleaned. (Photograph included) 5. The chairs have been sent away to be refurbished with a washable covering. Alternative seating has been added while we wait for the chairs to be returned. (Photograph included) (e) The hot water in the three toilets in question has been adjusted at the sinks to match the water temperatures of those three sinks to match the temperatures of the other sinks in the setting. (Professional verification included) As a preventative measure the maintenance person for the service has been given more responsibilities and more scope to raise issues with management. Another layer of checks to the existing role of the supervisor, management, and service provider have been put in place. Summary Comment In respect of the corrective actions taken documentary evidence was submitted to the office of the Early Years Inspectorate, reviewed by the Early Years Inspector and deemed to meet the regulatory requirement
