#### Inspection of 8 June 2026 — Inspection Report

##### Regulation 9 — Management and recruitment

- (a) (b) There were no validated references available for one adult who had access to children. (d) See Statutory Notice section in relation to Immediate Action Notice IAN 0301 served. Appropriate and suitable checks must be completed on adults who have access and are working directly with the children to adequately safeguard them

- (2) (a)(b) The adult was removed from duties until receipt of two reference which were verified. The Registered Provider has implemented a strengthened pre-employment verification process which will be completed by management prior to any new staff or work experience participant commencing within the service

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. The soft close mechanism of the door from the Senior Wobbler room to the sanitary area was not operating effectively, causing the door to slam. This posed a potential finger trap injury. 2. The water temperature in the wash hand basin used by the children in the sanitary area off the Playschool room exceeded the recommended temperature of 43oC. A temperature of 46.9oC was recorded by the inspector at 10.49am. This posed a potential scald risk to the children. 3. One of the 17 garda vetting disclosures reviewed was not dated within the required three-year timeline. This was not in adherence with the Early Years Inspectorate Regulatory Notice ‘EYI-RN12.3 Renewal of Garda Vetting’. Safe Sleep: The following practices were not in line with safe sleep practice: 4. Children aged between one to two years were observed to sleep in cots with soft toys and comforters. This can pose a potential risk should the items cover the child’s face while sleeping. 5. The following were not suitable sleep facilities for children: • Two children under two years old were observed to sleep on low stackable beds. Children under the age of two years who do not sleep in a cot must have access to an appropriate floor bed with a mattress depth of between 6 to 15 cm. 6. A documented sleep risk assessment had not been carried out prior to children under the age of two years sleeping on low beds. An assessment of the potential hazards in the care room while children sleep on low beds should be carried out prior to sleep-time in order to mitigate any potential risks. These were not in line with current safe sleep guidance or the service policy on safe sleep. Action submitted by the Registered Provider

- General Safety: 1. It was established that this occurred because of a vacuum effect that can cause the Senior Wobbler Room door to slam. As an interim measure, staff were reminded that the fire door leading to the sanitary area between the Senior Wobbler Room and Preschool Room 1 must always remain closed to prevent this vacuum. The service has contacted a contractor to arrange to a permanent solution to the issue. 2. The service report that the thermostatic mixing valve (TMV) has been replaced and that staff will continue to complete daily risk assessments and monitor water temperatures to ensure they do not exceed 43°C. 3. The registered provider confirms that the renewal Garda Vetting application for this staff member was submitted on 2 June 2026 and is currently awaiting completion. Evidence of the application submission was provided to the regulator on the day of the inspection. A full audit of Garda Vetting renewal dates has been completed by the PIC and DPIC, confirming that there are no outstanding renewals. To prevent recurrence, the PIC and DPIC now have full access to the HR database, including Garda Vetting renewal dates, and quarterly audits will be conducted to ensure ongoing compliance. Safe Sleep: 4. Evidence was submitted that staff were immediately reminded that comfort items may be used to support a child in settling; however, they must be removed once the child has fallen asleep, in accordance with safe sleep practices. A staff memo was issued, and the Tusla Safe Sleep Guidelines were circulated to all staff. 5. Evidence was submitted that two new suitable floor beds have been purchased. Staff have been reminded that children under two years of age must be placed in either a standard cot or a foldable cot or the floor beds where staff determine that a cot may not be the most appropriate sleeping arrangement for a child. 6. Evidence was submitted that the daily risk assessment for the Senior Wobbler Room has been reviewed and updated to include a dedicated sleep risk assessment section. This will be completed before each sleep period, and all relevant staff were informed of the updated procedure and their responsibility to ensure compliance

**Immediate action notice.** An Immediate Action Notice was issued to the registered provider on the 18 February 2025 under Regulation 23, in relation to a non-compliance identified under Regulation 23. A response was received from the registered provider which mitigated the risk identified on the 21 February 2025. See body of report for details. Part III – Management and Staff

##### Regulation 9 — Management and recruitment

- (d) International Police vetting was not available for one adult where a review of documentation indicated they had resided in a state other than Ireland for more than six months as an adult

- (2) (d) The staff member has applied for vetting from the relevant country. The registered provider will ensure compliance in this area for any future staff recruited. The recruitment policy has been shared with administrator

##### Regulation 16 — Record in relation to pre-school service

- (1) (k) The registered provider did not ensure a full record in writing for accident and incidents was maintained. From a sample of 20 records reviewed; 10 did not record the signature of the parents acknowledging they had been informed of the incident, as outlined in the service’s accident and incident policy

- (1) (k) Reports that were identified as not signed by the NOK, parents have been notified by staff and parents encouraged to sign same. Staff in the service were reminded to encourage parents to sign the report. A copy of the incident and accident policy has been shared with parents which stipulates their obligation to sign the form

##### Regulation 19 — Health, welfare and development of child

- (1) (b) The transition to sleep was not observed to be timely or organised in the Nursery room. Children who were observed to signal signs of tiredness such as crying and some upset at 11.36am were not placed to sleep until 12.05pm. Staff were observed to verbally acknowledge that the children were tired and other transitions such as dinner and nappy changing were observed to take place during this period which further delayed the transition to sleep. Transitions should be organised and planned in order for children to have seamless pleasant experiences throughout the day. It is acknowledged that staff provided comfort to the children throughout

- The registered provider will commit to a child led approach ensuring children’s sleep needs are met and that routines should be altered to suit the needs of children transitioning to the service. This was shared with staff at the staff meeting

##### Regulation 23 — Safeguarding health, safety and welfare of child

- Infection Control: The following increased the potential risk of infection: 1. Hand washing practices were not adequate to support appropriate infection control. The following was observed: o Children’s hands were not washed after nappy changing in the senior wobbler and nursery rooms. o A child’s hands were not washed after using the potty in the senior wobbler room. o Children’s hands were washed using cotton wool with a baby oil and water mix in the nursery room. This was identified as a non-compliance on the inspection held on the 15 April 2024 and actions put in place failed to prevent a recurrence. o Children’s hands were washed in a communal bowl and dried using a shared towel in the nursery room. 2. There was a build-up of dust and debris in the runner of the sliding door to the sanitary accommodation off the playschool room. 3. Adequate space of 50cm was not maintained between cots in the cot rooms 1 and 2 which is not in line with Health Protection Surveillance Centre (HPSC) Management of Infectious disease in childcare facilities and other childcare settings which states that cots or sleeping mats should be spaced at least a half metre apart for infection control purposes. Inadequate spacing of cots was identified as a non-compliance on the inspection held on the 15 April 2024 in relation to cot room 3. Safe Sleep: 4. The temperature of the “dark “cot room was not maintained between 16-20°C whilst a child under the age of 12 months was sleeping. A temperature of 23.1oC was recorded by the inspector at 10.23am. An Immediate Action Notice was issued to the service in respect of this non-compliance. It is acknowledged that steps were taken to address this, and the temperature had reduced to 21.1oC at 10.45am. Action submitted by the Registered Provider

- Infection Control: 1. The infection control policy was shared with all staff. Staff were reminded at the staff meeting of appropriate handwashing practices. There is a plan in place to install a low-level sink in the care room. 2. The debris was removed from the runner of the sliding door. Staff were reminded at the staff meeting of appropriate cleaning practices. 3. One of the cots in room 1 was removed and 2 smaller cots were ordered for room 2 to ensure adequate space between cots. Staff were reminded at the staff meeting of appropriate distance between cots. Safe Sleep: 4. The service opened windows to circulate fresh air, and a fan was brought into the room to assist with this. Thermostats in the sleep rooms have been reduced. A memo was circulated to all staff remised of safe sleep guidelines and required temperatures. The safe sleep policy was also shared with staff

##### Regulation 29 — Premises

- (d) There was a gap between the flooring and wall in the Playschool leaving the potential for a build-up of dirt and debris. It is acknowledged that works had been completed on the flooring throughout the premises since the last inspection

- The gap has been repaired, and staff were reminded at the staff meeting to bring any maintenance issues to the attention of management
