- One reference obtained in respect of one staff member from a source other than a past employer was not suitable as the source of the reference could not be considered independent or unbiased
- Police vetting was not available for one adult who had lived outside the State for a period of longer than 6 consecutive months
Provider's corrective action:
- One reference has been received and checked and verified. References must be from a reputable source and be independent or unbiased, checked and verified before a candidate is hired for employment
- We have made sure all parts of a candidates C.V. and experience is noted at the time of interview and reference check. All staff must obtain police clearance for any country they have lived in for a period of 6 consecutive months or longer

- (1)(3) The registered provider did not notify the Agency in advance of implementing changes to the service’s registered details. The person in charge on the service’s registered details does not work in the service. The manager confirmed that the previous person in charge left the service in August 2024. On the inspector’s arrival, the manager identified themselves as the person in charge
Provider's corrective action:
- A change in circumstances had been sent in to change in circumstances in January 2025. However, the approval hadn’t been received by the date of the inspection. Awaiting certificate of registration. A change in circumstances should be sent in prior to the change in the service
