Inspection of 11 February 2026 — Inspection Report

Regulation 9 — Management and recruitment

  • (2)(a) The references for one staff member were not validated. This is a recurrent non-compliance and is at variance to the registered provider’s assurances that “all staff validation forms” would be accessible in staff files. (d) Following a review of the 3 new staff files presented on the day of inspection international police vetting was required for 2 staff members who had resided outside the jurisdiction for a period of greater than 6 months as an adult. While it is acknowledged that vetting disclosures were available there was no English translation available
Provider's corrective action:
  • (2)(a) Reference check of the staff member has been completed and added to the file. A staff file check list has been created to ensure all required document are added to staff files pre- commencement of employment. (d) International police vetting for the two staff members has now been translated into English. A staff file checklist has been created to ensure all required document are added to staff files pre-commencement of employment

Regulation 11 — Staffing levels

  • (1) During the period of inspection there were not an adequate number of adults working directly with the children attending the pre-school service. (2) The following adult: child ratios which were observed in the Toddler Room during the inspection were incorrect: There were 7 preschool children (aged 2 years) attending this room with 1 staff. Two adults were required at all times to satisfy the requirement of this regulation
Provider's corrective action:
  • (1)and(2) One Child has moved to the preschool room reducing the ratio in the toddler room to 1:6. The child fully transitioned to this room on Friday 13/03/2026 and the decision was made in partnership with the child’s parents, following careful consideration and review of the child’s stage of development. The ratio in the preschool room is now 22:2. The registered provider is continually striving to recruit new additional staff members. The service will reduce the number of childcare places for next year should it be unable to fill positions to ensure sufficient staffing levels are within compliance requirements to meet the needs of all children

Regulation 19 — Health, welfare and development of child

  • 1. Three children aged between 10 and 15 months from the Baby Room were observed to be fed lying back in bouncy chairs. This is an inappropriate practice for feeding children of this age. It was further noted that there were highchairs available for feeding the children in the service. 2. One child was observed to climb over the 3 bouncy chairs which were lined up at the wall denying access to the wall mounted caterpillar. The child was lifted away instead of the bouncy chairs being removed to allow the child access to their chosen activity
Provider's corrective action:
  • The registered provider stated 1. Going forward all staff are to be made aware of children’s stage of development and to encourage and support children to move from highchairs to low chairs and tables for feeding. Bouncy chairs will only be used for comfort and relaxation . 2. The bouncy chairs have been removed to provide full access to the caterpillar. All staff members to ensure children have easy access to play equipment and that play items are not obstructed by chairs/furniture

Regulation 22 — Food and drink

  • 1. There was no evidence of a varied and nutritious menu for those children attending the service on a full- time basis. It was reported by a staff member that the children in the Baby Room usually got the same dinner every day i.e. mash potatoes with soup made from frozen vegetables and packet soup mixes blended. It was stated they do get a change to a bolognaise dinner on the day that it is made for the rest of the children attending and it is served with potato. 2. Although it was verbally reported a weekly menu was available for the older children - which consisted of either, pasta and mince, potatoes, fish fingers and beans, soup and potatoes or sausages and potatoes, no menu was available for review. In addition these meal options offered lack protein and are high in salt content
Provider's corrective action:
  • 1. The menu has been updated to include different meals provided daily. All babies, toddlers , preschool and afterschool children have equal access to the dinner provided each day. Babies are provided with a variety of hot cooked nutritious dinners. All educators will monitor children’s food intake plus preference for meals and inform management so that menus can be adjusted and alternatives provided to ensure intake of all food groups. 2. The menu is now available and placed on the notice board where families can view it. The soup mix previously used has now been replaced with salt free stock to mix with vegetables. Management and cooking staff have reviewed the nutritional standards for Early Learning and Care Services to ensure they are followed

Regulation 23 — Safeguarding health, safety and welfare of child

  • The Inspectorate is not assured that adequate steps have been taken by the registered provider and staff to ensure the health, safety and welfare of the pre-school children attending the service based on the following observations made during the inspection: General Safety: 1. There was a bottle of cough medicine sitting on top of the toy kitchen unit in the Preschool Room which was potentially accessible to the preschool children. This was a health and safety risk. 2. There was a staff handbag observed sitting on top of a box at a low level in the lobby of the sanitary accommodation off the Preschool Room. This was potentially accessible to any preschool child who accessed the toilets independently and therefore a health and safety risk. 3. In the Preschool and Toddler Rooms there were sharp unprotected corners on the low-level shelving units and on one of the tables used in the Preschool Room. 4. The large notice board on the wall in the Preschool Room was not adequately secured to the wall. Two children sitting underneath it for their lunch were observed to pull it out from the wall at the bottom. This was a potential impact and injury risk. Infection Control: 5. The following cross infection risks were observed in the Toddler Room during handwashing: • 7 children were observed to wash their hands in a communal basin of water. • A grey cloth towel was observed in use by the 7 children for hand-drying. 6. There was no system in place for the laundering and management of linen in the service for cots that are shared. For example: it was observed that no cot linen was changed in cots that were shared between children attending the Baby and Toddler Rooms. When questioned a staff member confirmed this and stated that “there was no spare linen available”. This is at variance with the services safe sleeping policy that states “children will have individual beds with their own blankets and sheets”. 7. The lobby of the sanitary accommodation off the Preschool Room was used for the storage of toys and equipment. This is an infection control and cross contamination risk. 8. The perishable lunches items brought in by the children were not removed from the children’s bags and stored appropriately in the fridge. This is an infection control risk. 9. The ‘lion’ flip lidded bin observed in use for the disposal of used hand towels in the nappy changing area was considered unsuitable. This required the staff and children using the bin to touch the lid multiple times which is a cross-infection risk. Safe Sleep: 10. In the Baby Room during sleep time, sleep observations were not always documented by the staff member carrying out the physical observation e.g. the registered provider who was present in the care room was observed to document the 10-minute physical sleep observations which was carried out by another staff member. 11. The room temperature was not documented as part of the 10-minute physical sleep observation. Additionally, the staff carrying out the sleep observations did not ‘check’ the room thermometer that was located on a high shelf
Provider's corrective action:
  • Corrective & Preventive Action General Safety: 1. The bottle of cough medicine was removed and all medicines are clearly labelled with child’s name and stored in the medicine box in kitchen or on high shelf in care room. A staff meeting was held on the 10/03/2026 and the Administration of Medication Policy was reviewed with all staff members. 2. The staff members handbag was removed from the lobby area of sanitary accommodation off the Preschool Room. All staff notified that personal handbags and belongings must not be left in rooms or lobby areas accessible to children. 3. All sharp unprotected corners have now been covered with Safety Corner Guards in the toddler and preschool room. The risk assessment for both the toddler room and preschool room has been updated to include ‘All sharp Corners with edges covered’ to be checked daily and replaced as required. 4. The two large notice boards in the preschool room have now been fully secured to the wall. All staff have been reminded to continuously be aware of maintenance issues/hazards and report to the management immediately. Management will ensure maintenance issues are corrected in a timely manner. Infection Control: 5. All staff members were informed that children’s hands are to be washed at the sink and hands dried with their own individual disposable hand towel at all times, including after engaging in sensory play experiences. The hand-washing policy was reviewed and updated. A staff meeting was held on the 10/03/2026 and the Hand-Washing Policy was reviewed with all staff members. 6. All staff have been informed that there is always a sufficient stock of clean laundry in the hot press at all times. Children’s individual laundry boxes are clearly labelled where their individual bed-linen is stored. Safe sleep Policy was reviewed and updated . On the 10/03/2026 a staff meeting was held, where the Safe Sleep Policy and Infection Control Policy was reviewed with all staff members. 7. The lobby area of the sanitary accommodation off the preschool room has been fully cleared of equipment and toys. Staff have been informed that toys and equipment are no longer allowed in this lobby area. The Infection Control Policy was also reviewed with all staff on the 10/03/2026. 8. All perishable lunches brought in by the children are removed from their bags when they arrive in preschool. They are labelled with their name and stored in the fridge. All staff reviewed the health and safety policy on the 10/03/2026 to ensure that all staff follow correct procedures in relation to storage of food. 9. The flip-lidded bin has been removed from the bathroom. The registered provider has ensured that any bins bought for the preschool service in the future, will be a foot-operated pedal bin to prevent touching the lid of the bin and the risk of cross-infection. Safe Sleep: 10. The staff member who completes the 10-minute physical observation of children sleeping, documents details of their observation. The Safe Sleep Policy was updated and reviewed by all staff during a staff meeting held on the 10/03/2026 for the purpose of ensuring best practise going forward. 11. All staff now check and document the room temperature as part of the 10-minute physical sleep observation. The Safe Sleep Policy was updated and it was reviewed by all staff during the staff meeting on the 10/03/2026 to ensure best practise going forward

Regulation 29 — Premises

  • (d)There were 2 holes in the ceiling of the sanitary accommodation off the Preschool Room
Provider's corrective action:
  • (d) The two holes in the sanitary accommodation in the pre-school bathroom have now been filled. All staff were reminded at the meeting on the 10/03/2026 that any hazards /and or maintenance issues are to be reported to senior management immediately

Regulation 9 — Management and recruitment

  • (2)(a) and (b) Four written references were not validated. (4) One member of staff did not have documentary evidence of having a minimum Level 5 childcare qualification. A second staff member while it is acknowledged that they had documentary evidence of a childcare qualification there was no letter of equivalency available to state it met the minimum Level 5 childcare qualification. Both members of staff were observed working directly with the children
Provider's corrective action:
  • (2) Four validated references are now readily available and have been added to the staff file. The registered provider will ensure that all staff validation forms are easily accessible in the staff files. (4) The registered provider has confirmed that one staff member’s role includes cooking and administration and does not work with the children. The registered provider is still in the process of verifying the other staff members childcare qualification. Staff will be asked to ensure that they provide the correct documentation as proof of their qualifications and to verify their qualifications with the DCEDIY before commencing employment in the creche

Regulation 15 — Record of pre-school child

  • (1)(a) - (i) On Day 1 of inspection in the Preschool Room it was observed that there was no documentation or registration form pertaining to 1 child attending this room. Staff when questioned only knew the child’s first name
Provider's corrective action:
  • (1)(a)-(i) The child’s document was in the setting on Day 2 of the inspection for the Inspector to review. Ensure all children have a registration form in the setting prior to starting. Staff are to be encouraged to be familiar with the completion of these forms and the personal information that they contain. Summary Comment A completed registration form was reviewed on Day 2 of inspection. The registered provider’s response was accepted and the non-compliance addressed. This will be for review at the next inspection

Regulation 16 — Record in relation to pre-school service

  • (1)(h) On Day 1 of inspection when the inspectors reviewed the children’s attendance records at approximately 10.30am there were no details of attendance recorded for any of the children in the Preschool Room
Provider's corrective action:
  • (1)(h) Ensure that the staff complete the attendance record as the children enter and leave the setting. The registered provider will conduct spot checks on a regular basis to ensure the attendance records are being filled in correctly. Summary Comment The registered provider’s response is accepted and the non-compliance addressed. This will be for review at the next inspection

Regulation 23 — Safeguarding health, safety and welfare of child

  • The Inspectorate is not assured that adequate steps have been taken to ensure the health, safety and welfare of the pre-school children attending the service. The following observations were made: General Safety: 1. The floor mat in the cosy area of the Toddler Room was curling up at the corners and a trip hazard. 2. The glass patio doors in the Baby Room had stickers positioned at the top of the doors rather than at the child’s level at the bottom of the glass so they are visible to the children thus preventing an impact hazard. Infection Control: 3. In the Preschool Room the staff member observed changing a nappy, did not wear an apron and did not remove their gloves after nappy changing when washing the child’s hands. This is an infection control risk. Safe Sleep: 4. On day 1 of the inspection, physical safe sleep observations and a signature of the staff member carrying out the observations, were not documented on the sleep observation chart, when the inspector reviewed the record at 10.50am. A child had been put down to sleep at 10.30am and subsequently the staff member then ‘backfilled’ the information when the inspector reviewed the records again at 11.20am. 5. There was a fleece blanket hanging over the side of 1 cot which the registered provider stated was used to stop the children in cots from seeing each other. This is a safety and smothering hazard if pulled down by the child. Action submitted by the Registered Provider

  • General Safety: 1. The mat in the toddler room has been stuck to the floor to ensure the corners do not curl and cause a trip hazard. Check the corners of the mat when carrying out the risk assessment each morning. 2. Extra stickers have been added to the bottom half of the patio doors in the baby room. Aside from induction training the registered provider will continue to ensure the nappy changing policy will be reviewed on a regular basis. Management will carry out regular spot checks on nappy changing. Safe Sleep: 4. Review safe sleeping policy with all staff members. Review safe sleep policy with staff on a regular basis. 5. Fleece blankets have been removed from the cot rooms. Cellular blankets only in use. Only cellular blankets are available in the setting

Regulation 29 — Premises

  • (c)The light fittings located directly over the cots in both sleep rooms, in the hallway and in the sanitary area were not fitted with a protective covering

  • (c) New light fittings have been installed in the baby room, hallway, cot rooms, sanitary area and preschool room. The new light fittings are fitted with a protective cover

Inspection of 23 November 2023 — Inspection Report

Immediate action notice. A safety Immediate Action Notice was issued on day 1 of inspection 16/11/23 in relation to raised radiator surface temperatures which were a potential burns risk. A satisfactory response addressing the non-compliance was received from the registered provider on the 17/11/23.

Regulation 9 — Management and recruitment

  • (2)(a) The following was noted in respect of staff references. • One staff member and the student in the service did not have 2 written and validated references. • One staff member required a 2nd written and validated reference. • Three written references required validation. (4) There was no documentary evidence available for inspection to show that a 7th staff member had a minimum Level 5 childcare qualification
Provider's corrective action:
  • (2)(a) References have been received and validated for all staff and students since the inspection took place. (4) The 7th staff member has no childcare responsibilities in the service, their tasks include cleaning and some clerical work therefore do not need a minimum Level 5 qualification. The registered provider will ensure that all staff and student’s references are provided and verified before staff start in the service. This staff member is not counted in staff ratio and does not care for the children in the setting. We ensure our ratios are correct at all times

Regulation 19 — Health, welfare and development of child

  • The registered provider did not ensure that each child’s learning, development, and well-being was facilitated within the daily life of the service in relation to the following: BASIC NEEDS: 1. The consistency of some of the dinners was inappropriate for the age and stage of development of the children. For example two children aged 2 years 7 months were observed to receive a pureed dinner. Programme of Activities 2. There was no evidence of curriculum planning in the Baby Room. This was further confirmed by the registered provider who stated that “there was no curriculum plans at the minute and they were going on training”
Provider's corrective action:
  • Corrective and Action 1. The registered provider stated that dinners have a variety of textures, sometimes children prefer their dinner to be mashed. These children do get a variety of food with different textures at other times of the day such as toast in the mornings and during snack times. 2. The registered provider contacted an outside agency and had a visit from their Development Officer. The room leader was provided with many different ideas how to support the developmental needs of the child through sensory play, songs and other activities. Treasure baskets have been created and staff have reviewed the equipment and toys in the room to ensure they meet the age and stage of the children’s development. Curriculum plans are being filled out each week. Preventive Action 1. Ensuring children have a variety of textures in their snack. Provide parents with healthy, age-appropriate snack ideas that can be sent in. Encouraging children to try different foods at dinner and snack time. 2. Review curriculum plans regularly and provide staff with support in completing the plans. Encourage staff to complete regular continuous professional development

Regulation 23 — Safeguarding health, safety and welfare of child

  • The Inspectorate is not assured that adequate steps have been taken to ensure the health, safety and welfare of the pre-school children attending the service based on the following observations made during the inspection: Infection Control: 1. The children’s hands were not washed after nappy changing and one staff member did not wash their hands after carrying out a nappy change. 2. Ineffective hand washing practices were observed. For example; a staff member used only cold water and no soap to wash their hands. 3. Two staff members were observed carrying out nappy changes and did not wear a disposable apron. In addition, the requirement to wear disposable aprons was not detailed in the services nappy changing policy as per best practice guidelines. 4. The polythene gloves observed in use for nappy changing were unsuitable as per best practice guidance. 5. When questioned about the cleaning process for toys in the service, the staff member stated to the inspector that a chorine-based cleaning product and water was being used to clean the toys and equipment. This is at variance with best practice guidelines. General Safety: 6. The surface temperatures of the radiators in the service ranged between 52.7 – 64o C in the sleep rooms, Preschool Room, sanitary accommodation off the Preschool Room, the Baby and Toddler Room which exceeded the safe radiator surface temperature of 50o C. This is a burns risk. 7. The 4 metal posts of the gazebo in the covered area in the outdoor area were unprotected and a potential injury hazard. 8. In the Preschool Room the CD player was unsecured, it had a trailing wire and was accessible to the children. This is a potential impact and safety hazard. 9. In the Preschool Room antibacterial spray was left on the low-level windowsill and accessible to the children. This is a safety risk. Safe Sleep: 10. A fleece blanket was observed in use in the sleep on a sleeping child under 1 year of age. Action submitted by the Registered Provider
Provider's corrective action:
  • Infection Control: 1 and 2. We have discussed during our staff meeting the importance of hand washing for staff and children. We have reviewed the hand washing and infection control policies and procedures with all and ensured everyone understands the importance of hand washing. 3 The use of disposable aprons has been included in the nappy changing policy. 4 We have reviewed the gloves that were in use, we checked and ensured that the other gloves we have in use are in line with the Management of Infectious Diseases guidelines. 5 We will ensure staff clean toys will be cleaned with washing up liquid and warm water and then disinfected after this. General Safety: 6 This was replied to in the immediate action notice. The heating was adjusted, and the radiators monitored until they reached the appropriate temperature. 7 An upholstery company are making covers for the poles of the gazebo in the outdoor area. Children are monitored closely outside around the gazebo area to ensure their safety. 8 The CD player has been moved to a higher shelf in the preschool room out of reach of all children. 9 The staff in the preschool room will ensure that the antibacterial is always out of reach of the children. Safe Sleep: 10 This blanket belonged to the child and is used to comfort the child. The blanket is removed and a cellular blanket is put in place when the child has settled and sleeping. Infection Control: 1. Management will review and assess all staffs hand washing techniques and ensure children’s hands are being washed. 2. Provide hand washing training to all new and current staff when they commence employment and annually. 3. Complete spot checks to ensure aprons are being worn during nappy changing and hand washing is being completed correctly. 4. Ensure the correct gloves are being bought and supplied. 5. Create an instruction manual for washing toys. 6. Checking the radiators temperature daily and recording it on the risk assessment form. 7. Children are monitored closely outside around the gazebo area to ensure their safety until the covers arrive. 8. The CD player will remain on the high shelf at all times. (see photographic evidence attached). 9. Basket with label has been put on the high shelf to store the antibacterial spray and cleaning products. Out of the reach of all children. (see photo attached) 10. Cellular blankets are left on the side of cots to ensure they are used instead of other blankets. (see photo attached)
