(1) It is acknowledged that of a sample of 12 records of a pre-school child, reviewed by the inspector, elements (a), (c), (d), (e), (f), and (g) were recorded. However, the inspector observed that element (h)-the immunisation status of each of 7 children was not detailed. The inspector also observed that the written parental/ guardian consent (i) for appropriate medical treatment of the child in the event of an emergency had not been recorded for 6 children
Provider's corrective action:
The registered provider submitted the following response. A new application form that has a section for the date of birth on it has been developed, and it also has another new section on it seeking parent/guardians consent to get emergency treatment for their child in case of an emergency in the setting. These have been given out to all parents to fill in. We also have checked the vaccination records and have reminded some families that we still need their child’s vaccination records for our files. We will ensure that all parents/guardians fill in all sections of our new application form when they are enrolling their child, and we will also ensure that we receive the children’s vaccination records before the child starts in our setting
Regulation 23 — Safeguarding health, safety and welfare of child
General Safety: The inspector observed that works had recently taken place around the boiler in classroom 2. While it is acknowledged that the boiler was inaccessible to the children, the inspector observed that the copper pipes coming from the boiler were left exposed and had not been enclosed. The pipes could potentially be accessed and touched by the children, possibly causing a burn injury if the pipes were hot. General Safety: The landlord has been asked to have the boiler pipes area covered and they hope to have someone do this over this midterm. The registered provider will ensure that all the necessary actions are put in place to make sure our setting is a safe environment for all children and staff
Regulation 25 — First aid
(1) There was no staff member present with in date first aid responder (FAR) training. It is acknowledged that one staff member’s accreditation for FAR had just expired on 18 December 2025 and the registered provider described how they had great difficulty in obtaining refresher training. It is acknowledged that the registered provider demonstrated to the inspector that FAR training and refresher FAR training had been obtained for the following dates 17, 18, 19 February 2026 and 5, 12, 19 March 2026
Provider's corrective action:
First Aid Responder (FAR) training and refresher FAR training have been obtained for the following dates 17, 18, 19 February 2026 and 5, 12, 19 March 2026. The registered provider will forward certificates of accreditation once the courses are completed
Regulation 26 — Fire safety measures
(1)(b) A record was maintained of the dates of checking and maintenance of the firefighting equipment and fire alarm system in the service, by a reputable contractor. While it is acknowledged that the last maintenance check for the firefighting equipment was compliant and recorded as having taken place on the 02 September 2025, the smoke alarm system had not been serviced since the last recorded date of April 2024

The registered provider submitted the following response. Corrective and Preventive Action The fire alarm contractor called to the setting week of January 26th and he serviced the alarms. He is going to post out the new certificate

Inspection of 28 August 2024 — Change in Circumstance
Immediate action notice. An immediate action notice was issued to the registered provider, on day two of the inspection process, as the registered provider could nor assure the inspector that there was always a staff member available in the service, with a current paediatric first aid responder training certificate. The registered provider responded to the immediate action notice on the 13 November 2023. A sampling process was used to assess compliance under regulation 15. Regulation 9(1)(2)(4)-Management and Recruitment, Regulation 10-Policies, Procedures etc. of a pre-school service, Regulation 11(1)(3)- Staffing Levels, Regulation 15(1)-Record of a Pre-School Child, Regulation 20-Facilities for Rest and Play, Regulation 25- First Aid, Regulation 26- Fire Safety, Regulation 28- Insurance. As a result, the scope of the inspection included both Montessori classrooms and the outdoor play area.
Regulation 10 — Policies, procedures etc. of pre-school service
While it is acknowledged that the following policies were available, they were observed by the inspector not to provide adequate details required and specific to the service. It is acknowledged that since the inspection, the registered provider engaged with a support organisation, to develop and revise the service’s policies and procedures. 1.The inclusion policy did not detail how the service recognises and responds to bias or discrimination. 2.The policy on outings did not include the following details, that a risk assessment is carried out prior to an outing and the checklist to be followed in preparation for an outing. The policy did not specify the adult / child ratio maintained while on an outing. Details regarding first aid provision, for example, assurance that an adult trained in first aid responder (FAR) training for children accompanies the group on an outing with an adequately supplied first aid box was not included. The policy did not detail any methods used for checking children while on an outing, for example roll calls. The policy did not provide assurance that for the duration of the outing, that a charged mobile phone was always accessible, with contact details for parents. The details of the procedure to be followed in the event of any critical incident that may occur while on an outing, for example if a child went missing, were not specified. 3.The policy on authorisation to collect children did not detail the routine for drop off and collection times. 4. The service’s policy on healthy eating did not provide sufficient information as to how the service ensures that sufficient, suitable, and nutritious food and drink are provided to the children while attending the service and what must be included in the snack, while attending the sessional service. The policy did not describe how food is safely stored and served, and how children are supported and supervised while eating and drinking. 5. The policy on use of the internet and photographic and recording devices, named in the service as the multi- media policy and procedure did not state that consent is required from a parent before the child is photographed and/or recorded. The policy did not include a sample consent form. The policy did not set out how photographic images or recordings are stored, details of how long they are retained in the service, how they are provided to parents or how they are destroyed. 6. The following policies were not available for inspection on both days, complaints policy, policy on accidents and incidents, policy on staff absences, recruitment policy, staff training policy and supervision policy
Provider's corrective action:
The registered provider submitted evidence of revised policies as required, specific to the service. The registered provider’s response stated that these policies will be revised on an annual basis and as required
Regulation 15 — Record of pre-school child
(1) While it is acknowledged that the inspector observed that elements of regulation 15 (1) (a) to (g) were clearly recorded in each of the preschool records, the following two elements (h) record of immunisations, if any, received by the child and (i) written parental consent for appropriate medical treatment of the child in the event of an emergency were not recorded
Provider's corrective action:
The registered provider submitted a response stating that the template for the record of a preschool child had been revised to include all details required under regulation 15(1)(a) to (i). Going forward, at registration of each child wishing to attend the service, all details will be obtained and accurately recorded
Regulation 25 — First aid
(1) On review of staff files, the inspector observed that one staff member had trained in first aid responder (FAR) training, however the certificate had expired on the 1 December 2022, and refresher training had not been followed up. An immediate action notice for safety, regarding not having a current qualified staff member in FAR was issued to the registered provider
Provider's corrective action:
The registered provider submitted a response to the Immediate Action Notice (IAN) stating that First Aid Responder (FAR) training had been booked for two staff members, commencing on the weekend of 25 November 2023, for three consecutive weekends.
