Five adults did not have a second written validated references on file. (3) The procedures as detailed under (2) (a),(b) were not carried out as required prior to the adults being appointed to work with the pre-school children - All outstanding references identified during the inspection have now been validated. This includes the verification of the reference from the past employer for one adult and the sourcing and validation of a second written reference for the five adults who did not have them on file. A full audit of all staff files has been completed to ensure that each file now contains two written, validated references in line with Tusla requirements. Any missing or unclear information has been followed up and recorded. The manager has been assigned responsibility for ensuring all references are secured, validated, and logged before staff files are approved. The induction policy has been updated to clearly require two validated written references for all adults before they are permitted to engage in work in The Children’s Project ##### Regulation 23 — Safeguarding health, safety and welfare of child - Infection Control: 1. Nappy changing practice was observed used gloves were not removed after changing children’s nappies posing a potential infection control risk. Action submitted by the Registered Provider - Infection Control: 1. All staff present were immediately reminded of correct nappy-changing procedures, specifically the requirement to remove soiled gloves before redressing a child to prevent cross-contamination. All staff have re-read and signed off on the policy on nappy changing and the correct sequence of nappy- changing steps, including glove removal, hand hygiene, and surface cleaning. The nappy changing policy has been reviewed and updated to clearly outline each step required for safe and hygienic practice, including when gloves must be removed and when hand hygiene must take place. A new step-by-step infection-control poster/infographic has been placed at the nappy changing station to remind staff of correct procedures during practice. This clearly outlines each step of the nappy changing process Found compliant: Regulation 11, 15, 19, 25, 26. #### Inspection of 20 September 2023 — Inspection Report Full report (PDF, Tusla) ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1.The monthly fire drill had not been completed by staff as per the service’s policy. The last recorded fire drill was completed on the 29 June 2023. Failure to complete a monthly fire drill may pose a risk to the safe evacuation of the children in the event of a fire on the premises. This non-compliance was found on the last inspection on the 6 May 2021. Infection Control: 2. Nappy changing practice was observed. Staff did not wash children’s hands after nappy changing. This was not in line with the service’s nappy changing policy and may pose an infection control risk. 3. The bin provided in the children’s toilet was not pedal operated. It had a swing lid posing a potential infection control risk. Action submitted by the Registered Provider - General Safety: 1. Following the inspection, the person in charge had a conversation with all staff and indicated that a fire drill must take place once a month. The fire drill roster for the service has been amended, ensuring that a fire drill is due to take place once a month and was shared with staff. In response, a fire drill was planned and took place within a week, on Friday 29 September at 12 o’clock. The designated person in charge was assigned responsibility for both conducting and recording monthly fire drills. This will be discussed at the next team meeting. Infection Control: 2. Staff have been reminded of the service’s nappy changing policy and have been requested to follow this policy. A simplified step-by-step nappy changing procedure list has been placed on the wall in the nappy changing room alongside the full, in -depth nappy changing policy. The designated person in charge or a named deputy will encourage staff to follow the service’s policy. Staff have been requested to re -read the nappy changing policy and to sign off on this by the person in charge. This will be discussed at the next team meeting. 3. To improve infection control, a pedal operated bin has been ordered online and will be placed in the children’s toilet. The previous swing lid bin has been removed.
