In the Wobbler Acacia room at 13:00 the staff member caring for the children did not go into the cot room to physically check the five children who were sleeping at this time. Instead, the staff member observed the children from the door. This posed a potential safety risk as it was not possible to check children’s breathing and colour from this position. Action submitted by the Registered Provider - Safe Sleep: We have amended the roster to ensure there is additional support allowing staff to enter the cot room to check sleeping children. Management will ensure there is always support available by amending the cover required when needed and staff have been informed to call management should any additional help be necessary Found compliant: Regulation 9, 11, 16, 19, 25. #### Inspection of 11 June 2024 — Inspection Report Full report (PDF, Tusla) ##### Regulation 23 — Safeguarding health, safety and welfare of child - Infection Control: Inspectors observed a number of practices which posed a risk of transmission of infection within the service: 1. Some nappy changing practices observed during the inspection were inadequate for infection control purposes and at variance with the service’s nappy changing procedure as follows; A staff member was observed to carry out a number of nappy changes without washing their hands between changes. The staff member was observed lifting the lid of the bin to dispose of waste. The nappy changing mat was not cleaned between nappy changes. Children’s hands were not washed following nappy changing. 2. The handwashing practices observed were inconsistent and at variance with the policy in place in the service as follows; a. While it is acknowledged that children’s noses were cleaned promptly, a staff member was observed to clean a child’s nose and did not wash their hands following this. Another staff member was observed to clean a child’s nose and then to clean a second child’s nose without washing their hands in between. b. Children’s hands were not washed following outdoor play in the Toddler Cameroon room. c. Children’s hands were not washed before dinner in the Toddler Cameroon room. 3. The covering on a play tunnel in the Baby Acacia room was torn exposing the foam within which could not be effectively cleaned. 4. Sticky tape which had been placed around the sink in the children’s play kitchen in the Baby Acacia room was coming away and contained accumulations of dirt and debris which could not be effectively cleaned. 5. The ball pools in the Baby Acacia and Toddler Cameroon rooms were observed to have a build-up of dust and debris on the base of the pools. Staff stated that the ball pools are cleaned as part of the routine cleaning of the play equipment, however there was no record on the cleaning checklist in the rooms for the cleaning of the ball pools. Action submitted by the Registered Provider - Infection Control: 1. Staff have been retrained in the correct procedure for nappy changing, including picture of task analysis showing a step by step of the correct procedure for infection control. This includes handwashing, disposing of nappies and the cleaning of the changing mat. Continuous monthly training and monitoring by management team. 2. a.b.c. Training was given and documented with a training card to ensure our infection control policy on handwashing and when necessary to wash both staff/children’s hands during transitions. Continuous monthly training and monitoring by management team. 3. The play tunnel was removed from Baby Acacia and the room layout was changed to reflect this. Weekly checks of equipment to be checked by management and anything broken/ripped to be removed immediately. 4. The sticky tape was removed from the sink of the children’s kitchen area and cleaned. Staff are aware that sticky tape is not to be used going forward and to maintain the hygiene of this area. 5. Ball pool from Toddler Cameroon has been removed and a larger kitchen/creative area was made. The ball pool in Baby Acacia has a new cleaning schedule where it is cleaned thoroughly during sleep time and documented on the sheet. Daily cleaning for the ball pool in Baby Acacia.
