#### Inspection of 16 February 2026 — Inspection Report

##### Regulation 11 — Staffing levels

- (1) The registered provider did not ensure an adequate number of staff were available to meet the care needs of the children on day one of the inspection in a timely manner. The following was observed in the baby Acacia and toddler Serengeti rooms: o children who demonstrated a need for comfort could not be assisted as staff were attending to the needs of other children, o children were observed to engage in negative behaviours that required support which went unaddressed by staff as they attended to other children o transitions were prolonged which led to frustration for children. These are further detailed in the noncompliance section of Regulation 19

- An overall review of transitions, age and needs of the children took place, and a new routine was put place to ensure additional support from part time staff and management in particular rooms. Staff also took part in additional training where required. The Management team has increased in capacity adding additional support, close observation and supervision has helped the operations of the centre at specific times and staff are more aware what to do. Additional staff are also due to start in the coming weeks

##### Regulation 19 — Health, welfare and development of child

- 1. Staff struggled to respond adequately to care needs of children in the baby Acacia room on day one of inspection from12.15pm-12.29pm and again at 1.00pm-1.21pm. While one staff member was present children were observed to demonstrate clear signs of upset by crying and seeking interaction. Despite the efforts of the staff member, they were unable to meet the needs of all children at this time. It is acknowledged when brought to the attention of management arrangements were made to provide support. 2. Nappy changing practices in the toddler Serengeti room did not allow for a supporting one to one interaction as children’s peers repeatedly entered the sanitary area during the care practice. 3. Delayed lengthy transitions and a lack of planned activities in the toddler Serengeti room on day one of the inspection which resulted in children being disengaged leading to disruptive and challenging behaviours that on occasion went unobserved by staff

- 1. On the day of the inspection, while in ratio, the baby room had only recently opened and so the children were quite new to the centre and did require additional support for this reason. A second staff member was present in the room the following week and received training on meeting care needs and practices. The management team have been closely monitoring this room and staggering new children’s start dates. 2. Staff in Toddler Serengeti have been retrained on toileting/ care practices during nappy changing/ toileting times. The management team has been monitoring this closely and noted the approach is working better. 3. Staff in Toddler Serengeti has been retrained in curriculum planning and added a more structured daily routine to the children’s day. The management team will monitor this closely and supported staff change within the rooms

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Trailing flexes accessible to children in the wobbler Cameroon and toddler Serengeti rooms posed a potential risk of injury to children. Infection Control: 2. Handwashing practices in the toddler Serengeti room were at variance with the service policy and HSPC guidance that details handwashing should be conducted with running water and liquid soap. Children’s hands were washed using water and cotton wool before mealtime. This posed a potential infection control risk. 3. On day one of the inspection the table in the toddler Serengeti room was not sufficiently clean before mealtime to support environmental hygiene practices. This posed a potential infection control risk. 4. A build-up of dirt and debris was observed in the following areas which posed a potential infection control risk. o The nappy changing unit in the wobbler Cameroon room. o The nappy changing unit in the toddler Serengeti room. o The children’s toilet in the toddler Serengeti room. o The floor, wall and skirting by the bin the toddler Serengeti room. Action submitted by the Registered Provider

- General Safety: 1. The trailing flex has been enclosed. Staff are aware of the health and safety issues this can lead to and will report in their daily diaries. Infection Control: 2. All staff were retrained on our Infection Control Policy which includes handwashing. The management team will closely monitor this during their daily interactions in the rooms. 3. All staff was retrained on our Infection Control Policy that includes hygiene practices. The management team will closely monitor this during their daily hygienic practices. 4. A deep clean was carried out in all areas. The Management team have been doing more infection control checks throughout the day

#### Inspection of 26 August 2025 — Inspection Report

**Immediate action notice.** An Immediate Action Notice was issued to the registered provider on the 26 August 2025 under Regulation 23, in relation to a non-compliance identified under Regulation 23. A response was received from the registered provider which mitigated the risk identified on the 27 August 2025. See body of report for details. Part III – Management and Staff

##### Regulation 19 — Health, welfare and development of child

- 1. A child was observed to be upset throughout the morning of the inspection. Staff members in the care room were observed to offer comfort and support to the child at times; however, the child was observed to wander through the care room displaying signs of upset and crying at intervals during the inspection and did not receive the level of care required to adequately support them during a time of distress. It is acknowledged that the person in charge and deputy person in charge spent time in the care room, offering comfort to the child. 2. Children in the Toddler Zambia room were observed to play in an area of the garden that had limited equipment and resources, which impeded the children’s opportunities for play. A child was observed to enter an area of the garden which was separated by a plastic fence and bring toys back to the group. It is acknowledged that a staff member then brought additional toys to the area for use by the children. Toys and equipment should be readily available for use by the children

- 1. All staff have received updated training on care practices and how to support children who require additional comfort throughout the day. Management have been observing the staff interactions and monitoring the children’s wellbeing. 2. All staff have received updated training on the service’s play policy and how to support the children’s interests in the care rooms and during garden time. New equipment was also purchased. Management will continue to monitor garden play closely

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. The windows in a first-floor care room were observed to be opened without the safety restrictors while children were playing in the room at 11:10am and again at 03:21pm. Furniture was observed to be positioned underneath the windows, increasing the potential risk of injury to the children. An Immediate Action Notice was issued to the registered provider in relation to this. 2. Nappy bags were observed in a child’s low-level cubby in the Toddler Zambia room, which was accessible to the children and posed a potential risk of suffocation. Infection Control: 3. Two potties, which were stored in separate sanitary areas, were observed to have bodily fluids in them. This is at variance with the service’s toilet training policy which states that potties will be cleaned and disinfected after each use. The floor around one of the potties was observed to be wet with an unknown liquid, further adding to the potential risk of cross contamination. 4. A staff member was not observed to wash their hands after handling a potty which contained bodily fluids. It is acknowledged that the staff member wore gloves while handling the potty and disposing of the liquid. 5. A staff member was not observed to change their disposable apron or clean the changing mat between four nappy changes. 6. The children’s sanitary area in the Toddler Safari room was observed to be cluttered with a towel stored on top of the toilet seat. This poses a potential risk of cross contamination. 7. Children’s soothers were not stored hygienically in the Toddler Zambia room. 8. A visible build-up of dirt was observed in the following areas, posing a potential risk of cross contamination and reducing the effectiveness infection control measures: o The floor, door frames and baby gate in the Toddler Zambia room. o On the door frame outside the Preschool Kenya room. o On a changing mat and changing unit in the upstairs sanitary accommodation. Safe Sleep: 9. Two children were observed to wear bibs while sleeping. This posed a potential risk of suffocation. Action submitted by the Registered Provider

- General Safety: 1. Discussions, retraining and role play was carried out with the staff to ensure there is no potential risk of injury to a child and that the staff understand what corrective action to take should a similar situation arise again. The management team have been carrying out more safety checks throughout the day. 2. Staff in Zambia has been re-trained on the risk of placing nappy bags at a low level. Management and staff will monitor this so there is no recurrence. Staff will also use the safety check booklet in place. Infection Control: 3. Staff received updated training on the sterlisation and storage of potties and the areas that they are located. Management will monitor this closely during their daily interactions in the rooms. 4. All staff received updated training on the service’s infection control policy which includes handwashing. Management will closely monitor this during their daily interactions in the rooms. 5. All staff received updated training on the service’s nappy changing policy. Management will closely monitor this during their daily interactions in the rooms. 6. Staff received updated training on cross contamination. The room was thoroughly cleaned and staff understand this standard is to be maintained. Management team will closely monitor this during their daily interactions in the rooms. 7. Staff received updated training on our soother policy. Management will closely monitor this during their daily interactions in the rooms. 8. Staff and the centre cleaners have received updated training on infection control measures and the standards expected within the service. Management team will closely monitor this closely. Safe Sleep: 9. Staff have received updated training on the safe sleep policy. Management will closely monitor this during their daily interactions in the rooms

##### Regulation 19 — Health, welfare and development of child

- 1. Staff in one care room did not respond to a child’s cue for additional food at dinner time. Following an incident in which three children’s bowls were knocked onto the floor, one child was observed to point to another child’s bowl and attempted to verbally indicate they wanted more dinner however staff did not provide more food to the children. 2. The bookshelf in Wobbler Cameroon was observed to contain 18 books; 10 of which were in poor condition and a selection of loose pages from books. Children need access to books to support early language and literacy experiences

- 1. The staff team were retrained on the healthy eating policy to ensure children are offered an additional portion of food if required. The centre management will liaise with the team on a regular basis ensuring this practise is always maintained. 2. New books were resourced for the wobbler Cameroon room on the 05.03.2024. The staff along with the centre management team will ensure to maintain books in good condition and replace as needed to support early language and literacy experiences for the children

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Spray cleaner was observed in a low press accessible to the children in Wobbler Cameroon. It is acknowledged that the press is fitted with a child lock however this was not used on the morning of inspection. 2. Foam was torn on the corner of a pillar at children’s head height in the outdoor area. This posed a potential risk of injury should a child fall against the pillar. Infection Control: 3. The nappy changing procedure was observed to be inconsistent within the service and not in line with the service policy for example children were not observed to have their hands washed following nappy changing. Handwashing is required for infection control purposes. 4. Debris and crumbs were observed on the nappy changing mat in Wobbler Cameroon. This posed an infection control risk. 5. A build-up of residue and crumbs was observed around the safety gate at the entrance to the nappy changing area in Wobbler Cameroon. This posed an infection control risk. 6. Paper towels and dispensed hand soap were not available at the low sink in Wobbler Safari to support adequate hand hygiene. Action submitted by the Registered Provider

- General Safety: 1. All staff were retrained on the use of safety locks to ensure cleaning chemicals are stored out of the reach of children or in a locked press. As part of the opening checks each morning all press locks will be checked to ensure they are locked, and spot checked throughout the day. 2. The foam has been replaced on the corner pillar since the day of inspection. As part of the daily hazard checks management will report to the maintenance team should there be potential risks. Infection Control: 3. All staff were refreshed on the handwashing procedure along with the importance of when is it required for both staff and children. This will be closely monitored by the management team. 4. All staff were retrained on the importance of changing room hygiene to prevent risk of infection and cross contamination. This will be closely monitored by the management team. 5. All staff were retrained on the importance of base room hygiene to eliminate the risk of infection and cross contamination including the entry and exit points to base room & nappy changing area. This will be closely monitored by the management team and maintained by staff with the support of the centre cleaner. 6. All staff were retrained on the handwashing procedure and ensuring there are always adequate supplies available at the handwashing sinks. This will be overseen the management team and maintained by the staff and centre cleaner with supplies replenished as required
