##### Regulation 15 — Record of pre-school child

- Five of the 12 child records under review did not contain some of the following required information; (d) The names, addresses, and telephone numbers of an alternative contact, and information on how to contact an alternative contact during the operational hours of the service. (e) Authorisation for the collection of the child. (h) Record of immunisations, if any, received by the child

- (1) The registered provider stated that all forms were updated and completed correctly to include emergency contacts, authorised collections and immunisations, and that all forms will be checked by staff members

##### Regulation 23 — Safeguarding health, safety and welfare of child

- Infection Control: 1. Two bins in use by the service were not pedal operated posing a risk of transmission of infection. The bins had open tops and were observed with waste inside. Administration of Medication: 2. Documentation, procedures and practices in place for children attending the service requiring emergency medication were inadequate, posing a potential risk to the health and safety of children. There was no documented care plan available for one child attending the service who required emergency medication. Action submitted by the Registered Provider
