Sligo): what Tusla inspections found — 2 published inspection(s), non-compliances and the provider's corrective actions. ## Cre8ive Corner Creche Full Day · 0 - 6 Years · Sligo, Sligo · Tusla ID **TU2015SO008** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 13 October 2025 — Inspection Report Full report (PDF, Tusla) ##### Regulation 16 — Record in relation to pre-school service - (1) (h) On the day of the inspection, attendance was not adequately recorded in the wobbler room, as at 10:40 there were eight children present, but only five were signed in. (1) (k) A sample of accident and incidents records involving a pre-school child were reviewed for this regulation. The following information had been omitted from the forms. • The signature of the parent was not present on one form. • The manager’s signature was omitted from five of the forms - (1) All Staff have been informed to sign all children in as they come to creche. (2) All Staff have been informed to make sure all accident report forms are signed by parents and managers ##### Regulation 19 — Health, welfare and development of child - (1) (a) • The individual sleep needs of children in the wobbler room aged 1 year old were not met. One child was notably tired at 10.00am and were observed to wait up to until after dinner at 12.02 before being placed to sleep. The child was observed rubbing their eyes, crying and disengaging from play activities and unable to eat their dinner. The structure and routine of the day in this room indicated that children ate their dinner prior to being placed to sleep. This process did not allow for children to be placed to sleep when they were tired and in need of rest. • On the day of the inspection, the privacy and dignity of children were not respected. At 2:00 PM, a child’s nappy was being changed in the bathroom while six other preschool children were present - (1) The child on the day in wobblers was settling in. The child’s parents had known the child was upset and had requested the child have sleep at 12 as it is what she has as routine at home. All children have opportunity to sleep when they want which was observed on the day in toddlers. When parents ask about doing their routine, we do explain that we have to let the children sleep as they need. (2) To keep ratios the staff used to split the number of children between two staff to change nappies. We will now use the door so staff will bring one child at a time to changing unit to keep the child’s privacy. I have briefed all staff that they can use the sliding door in toddlers for changing nappies ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. On the day of the inspection, chemicals were observed stored on the fire alert panel in the main entrance hall and were not locked away, posing a potential risk to children's safety. 2. Whole grapes were observed in the preschool room without being cut, posing a potential choking hazard. It is acknowledged that when the inspector brought this to the attention of the staff members the grapes were cut. 3. Low level visibility strips were not present in the glass doors in the wobbler room posing a safety risk for the young preschool children. 4. A wooden kitchen in the wobbler outdoor area with a broken door, was posing as a risk to children. 5. In the outdoor area of the wobbler room there was a broken part of the gate and there was exposed sharp edges and posing a risk of injury to the preschool child. 6. In the rest area of preschool one soft furnishings were ripped with exposed foam which if ingested could injure the preschool child. Infection Control: 7. Hand hygiene was not completed for children after nappy changing, which could lead to cross- contamination. 8. The wooden tables in the preschool room were observed to be worn and in poor condition, making them unsuitable for effective cleaning. Administration of Medication: 9. No evidence of parents being informed of administration of prescribed medication which may lead to overdose. 10. Medical risk assessment had instructions indicating that a care plan should be in place. However, staff were unable to locate the care plan when requested. While it is acknowledged that the staff member present was aware of the steps to be taken should the emergency medication be required. This could pose a risk with unfamiliar staff. Action submitted by the Registered Provider - General Safety: (1) This is flower food that staff use for the flowers outside it was just purchased and not open. Going forward. the flower food will be always stored away in our storage area. All staff have been reminded of the need to store such item in a proper place. Another letter has been sent out to parents again about cutting their fruit. (3) We have added child friendly stickers to our windows in the wobbler room. Always keep stickers on the window. (4) We have removed the kitchen on the day of inspection. We will monitor all outdoor play equipment on weekly bases. (5) I had mentioned on the day of inspection that we are in the middle of getting new fence in this area. we have got new fence done. Gate and fence have been replaced. (6) On day of inspection, I have removed the cushion that was ripped. Weekly check list on soft toys & cushions. (7) Infection Control: (8) All staff have been informed that children’s hands must be washed after they get their nappy changed. I have put a post up beside each changing unit to remind the staff. A post has been put up at each changing unit to remind the staff to wash the children’s hands after every nappy change. (9) I have ordered new tables for preschool we have removed the wooden table, to stop this from reoccurring in the future we will do monthly check on all table and chairs in the rooms. Administration of Medication: (10) All parents are informed about medication they provide for their child. We fill out medication forms these are signed by parents. We were asked to get each dose of the medication signed on the form. All staff have been informed that each dose of medication must be signed individually by the parents. These will be checked by managers. (11) A care plan for the child has been got off the child’s parents. We will always have the care plan on the wall doe the child and all staff have been briefed on this ##### Regulation 29 — Premises - The registered provider did not demonstrate the premises are cleaned, maintained and repaired as follows: (1) Preschool Room 2 and 3 had evidence of flaking paint on the walls and woodwork of the care room and required repainting. (2) The taps in the sanitary accommodation of preschools 1, 2 and 3 were corroded with evidence of a rust like substance - (1) We have touched up all flaking paint. We will do checks on each room to see if the rooms need topping up. (2) All taps have been replaced with new taps Check taps on monthly basis Found compliant: Regulation 9, 11, 15, 20, 26, 27, 28. #### Inspection of 24 April 2024 — Inspection Report Full report (PDF, Tusla) ##### Regulation 9 — Management and recruitment - The procedures specified in paragraph (2) were not carried out prior to an adult commencing employment in respect to one adult in the service where an international police vetting was dated the 11.04.2024 and employment began in November 2023 - The service advised in a written response: We will ensure all vetting documents are in our creche office as well as our HQ office. Management has taken this on board, and it won't happen again ##### Regulation 19 — Health, welfare and development of child - Two children in the Wobbler care-room were provided with stackable beds that did not have bed sheets on them. It is acknowledged that new sheets were sourced by the service on the day - The service advised in a written response: Sheets will be always on the beds when children are on them regardless of wipeable beds or not ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. The outdoor metal gate was difficult to close, and the inspectors had difficulty on the day closing the gate on two occasions. This gate required attention as the risk of not being able to close would pose a risk of unauthorised entry or unsupervised egress from the service. 2. A wooden car in the bark area of the outdoor space used by the children from the Toddler care-rooms was damaged with splinters protruding from it posing a risk of injury to children. Infection Control: 3. The foot operated pedal bin in the second bathroom in the second sanitary area was broken and foot operated pedal bin in preschool 3 was not working, resulting in a risk of contamination. Safe Sleep: 4. There were a number of issues with cot mattresses in the sleep room used by the children attending the Wobbler care-room as follows: • One cot had a mattress that did not correctly fit the cot with a gap of 13cm between the cot and mattress frame. • A second cot had a mattress that had no safety mark or mattress cover in place. • A third cot had a mattress with no safety mark and the sheet covering the mattress had a hole in it. • A fourth cot had a mattress with no waterproof cover on it. Action submitted by the Registered Provider The service advised in a written response that: - General Safety: 1. Our maintenance man had a look at the gate, and it is now easier to open. The gate is metal and expands with heat. The service will monitor this on hot days, and sort an alternative if needed as security is our priority. 2. The wooden car handle has been removed from the back area. Infection Control: 3. The two pedal bins that needed to be replaced have being replaced. These bins will be checked regularly to ensure they are working. Safe Sleep: 4. The cot mattress that there were issues with have being changed and all replaced so they fit to size and have warning signs on them, and mattress covers were applied.
