The front main entrance door was not secured; this posed a risk of unsupervised exit by a child. This practice was found to be non-compliant on the previous inspection on 8 October 2024. The corrective actions submitted following those inspections failed to prevent recurrence of this non-compliance. Infection control: 2. On the day of inspection, the staff toilet was observed to be used as a storage area and contained a children’s vinyl chair, cloth cushions and pieces of timber. This posed an infection control risk. 3. The inspector noted that the outdoor sand pit was unable to be covered properly as the cover was in poor repair, posing a risk to the children from contaminants. This practice was found to be non-compliant on the previous inspection on 8 October 2024. The corrective actions submitted following those inspections failed to prevent recurrence of this non-compliance. It is acknowledged that the registered provider stated that they had replaced this cover since the last inspection. General safety: 1. Hooks and eye in place were removed, and an alarm was fitted to both doors. Infection control: 2. The items have been removed, and a sign has been put in place. 3. Summary Comment The actions taken by the registered provider have addressed the non-compliance identified under Regulation 23 ##### Regulation 29 — Premises - (e) Thermostatically controlled hot water was not available in the both the children and adult sanitary areas on the day of inspection. Water temperatures were recorded between 14.6°C and 15.4°C - The following statement was received from the registered provider: Corrective and Preventive Action A water heater has been installed Found compliant: Regulation 9, 11, 15, 19, 21, 24. #### Inspection of 8 October 2024 — Inspection Report Full report (PDF, Tusla) ##### Regulation 15 — Record of pre-school child - (g) Five general practitioner telephone numbers were missing from five children’s Pre-school records. Not having general practitioners telephone numbers completed for each child may delay care or compromise the safety of the children being cared for by the service - The following statement was received from the registered provider: - (g) The missing general practitioner’s telephone numbers have been added to each child’s pre-school record ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: (1) Two of the windows in operation in the care room were observed not to be secured to prevent accidents occurring. The inspector noted children climbing on the window sill, beside one of the open windows throughout the inspection, while staff in the outdoor play area were communicating with staff in the indoor care room through the open window. (2) A wooden toy box, in a poor state of repair, was accessible to the children walking to the outdoor play area, posing a safety risk should a child catch their hand/fingers on the splintered wood on the box. (3) The mud kitchen sink was filled with dirty water and the resources in use were covered in dirt and left lying on the ground. In discussion, staff were unable to state the last time the water had been replaced or the area had been cleaned, posing a general safety risk from possible contaminants in the water which staff stated children use for play. It was acknowledged that when informed a staff member removed the water and cleaned the area. In addition, there was no cleaning schedule available or risk assessment in place for the outdoor area. (4) The cover for the sand was observed to be embedded in the sand and the material had perished. In conversation, staff stated that the sand box is left open, posing a risk to the children from contaminants. (5) The main entrance door was not secured from the inside. The door, which leads to the school playground, was operated by a push bar mechanism located at the children’s level which was easily accessible, posing a safety risk should a child leave the service unsupervised. Infection Control: (6) Contrary to the service’s infection control policy which stated “..that staff understand that maintaining a good standard of environmental hygiene is important” this was not found in practice on the day. The inspector observed that the children’s sanitary areas were generally unclean; toilet bowls were stained, one wash hand basin had sand and mud residue around both taps and the window of one of the toilets had spider’s webs on it. Not ensuring daily cleaning increases the risk of infection and cross contamination. (7) The service maintained a daily tick sheet to evidence that daily cleaning indoors was being carried out. The manager stated that although the cleaning schedule is ticked as being completed each day, cleaning in fact only takes place every second day and on each Friday. Maintaining false cleaning records poses a risk to the children attending the service. (8) In contrast to the waste section of the service’s infection control policy which stated “foot operated pedal bins are used to dispose of disposable gloves and aprons” the inspector observed a swing bin in operation in the nappy changing area of the adult toilet. It was jammed open with a disposable apron and gloves sticking out the top, posing a risk of cross contamination from the adults touching the lid to insert the disposable items and from touching the used items that were preventing the lid from closing properly. (9) One pedal operated bin, outside the children’s toilet area, was broken preventing the lid opening and closing automatically, posing a risk from cross contamination as the inspector observed multiple children opening the lid themselves, with their hands, to dispose of used paper towels after handwashing. (10) A leather couch, in use in the quiet/rest area, was noted to have a large area of exposed wadding and foam, making it difficult to clean effectively. Action submitted by the Registered Provider - The following statements were received from the registered provider: General Safety: (1) Window restrictors have been fitted to the windows to ensure the safety of the children in the service. (2) The wooden box was removed from the area in order to prevent any safety issues. (3) Staff cleaned the mud kitchen and the immediate area on the day, while the inspector was present. Staff were reminded at a recent staff meeting of the need to risk assess the area before the children go out to play and to ensure the area is cleaned after use. (4) A new cover has been purchased and staff have been reminded of the importance of ensuring that the sand area is covered when it is not in use to prevent contamination. (5) A safety device has been fitted on the main entrance door to alert staff in the event that a child attempts to leave the service unsupervised. Infection Control: (6) and (7) Staff have been reminded of the need to ensure that daily cleaning is carried out and that only those areas which are cleaned should be ticked on the cleaning schedule. In addition, the post of cleaner has been filled with the intention of assisting staff in ensuring the service is kept clean. (8) A small pedal operated bin for paper waste has been installed. (9) A new pedal operated pedal bin is now available outside the children’s sanitary area. (10) The couch in the rest/quiet area has been replaced with the new sofa which was on order ##### Regulation not named in the report text - (1) Through conversation with the manager, at the introductory meeting, it was found that the service had not informed the agency of a change in circumstances regarding a change in the designated person in charge, which had occurred in January 2024. Not ensuring this change had been communicated to the Agency, and updated on the national register, does not allow for accurate service operations details to be retained and published - This inspection was unannounced and focused on the area of governance/ health, welfare and development of child and safety. The inspection may also focus on other areas as required. Inspection findings are documented in the inspection report which is first issued in draft format to the service with an opportunity to respond to any findings. Where statutory requirements are identified as not being met, the registered provider must demonstrate how they have rectified the non-compliance and will prevent any non - compliance from re occurring. The Corrective Action and Preventive Action plan (CAPA) will be used to inform decisions about compliance with regulatory requirements. Where the registered provider fails to meet the statutory requirements an escalation process may be commenced. The inspectorate reserves the right to edit responses received for reasons including clarity, completeness and compliance with administrative and legal processes.
