Dublin): what Tusla inspections found — 2 published inspection(s), non-compliances and the provider's corrective actions. ## Castlefield Montessori School Sessional · 2 - 6 Years · Dublin 15, Dublin · Tusla ID **TU2015FL054** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 12 February 2026 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** 12 February 2026 An immediate action notice was issued to the registered provider in relation to two significant concerns identified under Regulation 23, Safeguarding the Health, Welfare and Development of Child. A response which adequately mitigated the two concerns was received on the 13 February 2026. Further details are available under general safety and administration of medication section under Regulation 23. ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. In the outdoor play area, there was a hole in the fence making this area unsafe and unsecure. This posed a significant risk if one of the children were to try to go out or put their hand through the hole into the back garden of another home. This is an area that cannot be risk assessed. An immediate action notice was issued on the day of inspection. A response which adequately mitigated this concern was received on the 13 February 2026. 2. The drain cover was observed to be partially broken on the driveway entrance to the service. This posed a potential risk of injury. Infection Control: 3. Children were not observed to wash their hands before snack time and after outdoor play time. Staff were observed to administer hand sterilizer on the children’s hands before snack time. This does not support adequate hygiene. Children should be facilitated to wash their hands under thermostatically controlled running water, with liquid soap and paper hand towels provided in line with HSPC guidance in relation to infection control protection measures. Administration of Medication: 4. There was a child aged between 3-5 years attending the preschool who had prescribed emergency medication in this school bag. There was a significant risk identified due to the following. o The serviced did not have a plan to identify triggers, symptoms or the steps to follow to safely administer medication and treatment if required. o The medication was stored loosely in the bag at an accessible level to other children. o The medication was out of date with an expiry date December 2025. An immediate action notice was issued on the day of inspection. A response which adequately mitigated the concern was received on the 13 February 2026. Action submitted by the Registered Provider - General Safety: 1. The hole has been repaired on 12 February 2026. The registered provider has stated the hole will remain closed. 2. The drain cover was replaced on 12 February 2026. The registered provider has stated the drain cover will be checked on a regular basis. Infection Control: 3. The children will wash their hands before lunch and after returning from the garden. The registered provider has stated the children will keep washing their hands at appropriate times. Administration of Medication: 4. The registered provider stated that the child’s parents have confirmed that the child no longer requires emergency medication. The registered provider has stated they will ensure that children’s medical information will be updated by the service and parents ##### Regulation 26 — Fire safety measures - (1) (b) Documentary evidence showed the smoke alarms had not been serviced since 24 October 2023. A similar non-compliance was observed on the last inspection 10 October 2023 which showed that the preventive actions provided by the registered provider has not been sustained - Smoke alarms and emergency lighting were checked on 18 February 2026. The registered provider has stated the smoke alarms will be checked on a regular basis Found compliant: Regulation 9, 11, 19, 21, 29. #### Inspection of 12 October 2023 — Inspection Report Full report (PDF, Tusla) **Immediate action notice.** 12 October 2023 An Immediate Action Notice was issued to the service within Regulation 23- The Sensory room temperature was above the required 18 to 220C. A response which outlined the action taken by the registered provider to address the concern was received on the 13 October 2023. Please see details in the body of the report. ##### Regulation 11 — Staffing levels - (1) There was not an adequate number of staff working directly with the children on the day of inspection. Evidenced by the following observations: • During 10.45am-10.55am one staff member was observed to leave the Montessori room to go to help the other staff member in the toddler room for nappy changing time. During this time, there was one staff member with seventeen children. This was during snack time where it was observed by the inspector that some of the children required extra support and supervision. • The children in the toddler room did not get to go outside during the morning session on the day of the inspection. The staff member confirmed that the children usually get to play outdoors but there was no staff member available to assist the children go down the stairs on the day of the inspection due to a staff member being on leave - (1) The registered provider has confirmed that the reason for the low staff numbers on the day of inspection was due to a staff member calling in sick. The registered provider has stated they are trying to obtain someone who is qualified and can stand in in to cover if a staff member is out sick. The registered provided has stated that all children will go outside daily. Summary Comment The actions stated by the registered provider address the non-compliances identified. The regulatory requirement has now been met for Regulation 11 Staffing Levels ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: (1) Temperatures were not maintained within the recommended ambient room temperature of 18 to 220C. An Immediate Action Notice was issued to the registered provider on 12 October 2023 in relation to the following. • In the sensory room temperatures were recorded at 25.80C and 280C between 11.27am and 12pm while children were playing in this room during this period. Infection Control: (2) The nappy changing mat in the upstairs nappy changing area was not in a good state of repair. It was ripped in two places which increases cross infection. Action submitted by the Registered Provider - General Safety: (1) The registered provider has stated that the windows were opened, blinds pulled down and a cooler has been put into the sensory room on 13 October 2023. Staff have been asked to follow the above procedures and to check the room temperature daily. Staff have been instructed to bring children to the other room which is cooler if the room temperature is not between 18 to 220C. Infection Control: (2) A new nappy changing mat has been purchased and the registered provider has stated the mat will be regularly checked for damage or wear ##### Regulation 26 — Fire safety measures - (1)(b) There was no documentation available to demonstrate when the smoke alarms were last serviced - The registered provider has stated that the smoke alarms have now been serviced by a fire alarm company on the 24 October 2023. The registered provider has stated that it has been arranged for the fire alarm company to carry out yearly service checks including providing certification to confirm that the inspection have taken place ##### Regulation 29 — Premises - (e) The premises was not equipped with adequate and suitable sanitary facilities. On the day of the inspection staff stated the upstairs toilet is used for both adults working in the service and the children attending. Children and adults require separate sanitary facilities. The regulatory requirement is that there is one toilet and hand wash basin for every 11 children in attendance - The registered provider has stated that the service had an engineer out to look at the problem. The engineer has proposed a solution which will rectify the issue. There is an ensuite off the toddler room upstairs. The engineer feels this room is big enough to put in an air lobby and a sliding door and add a toilet and wash basin. This room has an extractor fan in it already. The door can be locked from the inside to accommodate privacy.
