Monaghan): what Tusla inspections found — 2 published inspection(s), non-compliances and the provider's corrective actions. ## Carrickmacross Preschool Playgroup Sessional · 2 - 6 Years · Carrickmacross, Monaghan · Tusla ID **TU2015MN011** · Registered since 1 January 2026 An inspection records what inspectors saw **on one day**; services respond with corrective actions — [how to read this](/methodology.md). #### Inspection of 14 October 2025 — Inspection Report Full report (PDF, Tusla) ##### Regulation 9 — Management and recruitment - (2)(a) One staff member required a second written and validated reference - The staff member was asked to submit a reference which was then checked and validated by the manager. Targeted questions were asked to the referee before validating the reference. The registered provider will ensure a more robust approach to the references moving forward when hiring new staff and ensuring they are verified verbally- clear questions will be asked to ensure the registered provider is getting a clear picture of the candidate ##### Regulation 16 — Record in relation to pre-school service - (i)The staff rota present on the day of inspection was inadequate for example it did not specify the hours each staff member worked each day. (k) Four accident / incident forms were reviewed and 3 had not been signed by the manager in the service - (i)The registered provider has updated a new staff roster/ staff sheet – it states the ECCE hours worked each day, dated individually and the staff working are clearly visible. The new updated roster sheet has now replaced the old roster sheet. (k)The accident book has been co-signed and all staff were verbally made aware at the staff meeting that the book need to be co-signed immediately. This requirement will be a point of focus during new staff training to ensure ##### Regulation 20 — Facilities for rest and play - (1)(b) The soft area in Preschool Room 2 was inadequate as it consisted of a small wooden sofa with plastic seating and 3 small wooden chairs. This is not inviting for a child to go and relax or lie down on if they so wished or to take a break from activities - A small soft fold away sofa was purchased. This can also pull out into a soft bed type mattress. A rug and soft cushions were also added to the area to make it cosy and inviting. This will remain in the setting and all staff are aware that cosy inviting areas are now essential ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. There were a number of unprotected sharp corners in both rooms; for example on the brown shelving units and child sized white table which were a potential injury hazard to the children. 2. The children were observed to play on a mini trampoline in the outdoor area. These are prohibited in childcare settings and are a health and safety risk - Corrective & Preventive Action General Safety: 1. Self-adhesive furniture corner protectors were purchased and have been stuck on all unprotected sharp corners. The corner protectors are visible and all staff have been made aware to ensure they are replaced if they fall off. Furniture protector check added to the Daily Risk assessment. 2. The mini trampoline was removed from the service Found compliant: Regulation 11, 19, 26, 28. #### Inspection of 13 September 2023 — Inspection Report Full report (PDF, Tusla) ##### Regulation 16 — Record in relation to pre-school service - (i) There was no daily staff roster available on the day of inspection. It is acknowledged that the registered provider sent photographic evidence to the inspectorate of a staff roster the following day - The registered provider developed a weekly staff roster and sent photographic evidence of it to the inspectorate the day after the inspection took place ##### Regulation 19 — Health, welfare and development of child - (1)(a) There was no drinking water available and accessible to the children throughout the day of inspection. It is acknowledged that the registered provider sent photographic evidence to the inspectorate the following day of water stations that had been set up, which were in the care rooms and at the children’s height - The registered provider set up water stations in the care rooms that were accessible to the children and sent photographic evidence to the inspectorate the day after the inspection took place ##### Regulation 23 — Safeguarding health, safety and welfare of child - General Safety: 1. In the outdoor area, a wooden gate at the bottom of a metal staircase which led up to a room of the premises which was used by an organisation other than the early years service was open and accessible to the children, as the lock was broken. This presented a risk to the children as they could have become injured on the staircase or have gained entry into the room, if the door was unlocked. It is acknowledged that the registered provider sent photographic evidence of a padlock on the gate to the inspectorate the day following the inspection. 2. On reviewing the Accident and Incident Form book, not all report sheets were signed by adults working in the service, parents and dated. The copy of the report had not always been given to the parent/guardian of the child. This presented a risk to the children if the information was not accurately and fully communicated. Action submitted by the Registered Provider - General Safety: Wooden Gate The registered provider bought and fitted a new lock for the gate and sent photographic evidence to the inspectorate the day after the inspection took place. The registered provider has stated that a statement to assess the gate has been added to a checklist on the daily outdoor risk assessment. Accident and Incident Forms The registered provider has stated that staff training took place to ensure that all staff are aware of how to complete the forms accurately. The registered provider has stated that the room leaders will review the books to ensure that the forms are completed correctly. Summary Comment The corrective and preventative actions as stated by the registered provider have provided assurances to the Inspectorate that the non-compliances have been addressed.
