#### Inspection of 16 April 2026 — Inspection Report

##### Regulation 9 — Management and recruitment

- Written and verified references from a past employer were not available in respect of four adults. (d) See Statutory notice section in relation to Immediate Action Notice IAN1521 served. (d) International police vetting was not available for one adult who had resided outside the state for a period of six months and was not working directly with the preschool children on the day of inspection
- A review of documentation evidenced that the requirements of Regulation 9(2) had not been completed prior to 4 staff members being appointed, assigned, or allowed access to or contact with a child attending the preschool service as outlined above under point 2(a)(b) and (2) (d)
- Certification of qualification available for one adult who was working directly with the preschool children required to be deemed equivalent by the minister to a level 5 or above on the national framework of qualifications

- In a written response the person acting on behalf of the registered provider stated:
- The manager will ensure that all the employees references match the employees Curriculum Vieta and are verified
- International police vetting applied for and received. The service will ensure that all employees that have lived outside of the republic of Ireland get international police vetting from the country where they had lived and ensure that this person does not work with children of any age until they receive the garda vetting
- The manager will ensure that the above (2) (a) (b) are adhered to at all times when recruiting new staff The manager will check with the recruit who their referees are on their curriculum vitea while attending interview. The manager will get the employee to sign a headed page with the dates they were not employed for and the reason for the gap in employment in the timeframe
- The staff member has sent all information to the Department to certify the qualification. The manager to ensure that this is checked before employing new staff and ensuring to check the qualification under the early years framework

##### Regulation 16 — Record in relation to pre-school service

- On the day of inspection, inspectors observed that a person trained in first aid for children was available during the hours of operation. However, the record of the staff roster was incorrect and did not highlight the availability of staff members trained in first aid for children for the duration of the operation of the service

- The roster has been amended to ensure there is someone available with first aid training at all times. Training has been arranged for six staff members

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General Safety: 1. Detergents and a bait box were stored in an unsecure cupboard accessible to preschool children in the toddler sanitary area posing a risk of harm if accessed by preschool children. 2. Flexes and chords from air conditioning unit was loose and accessible to the preschool children in the toddler room. 3. The radiator cover in the upstairs preschool room was not secured to the wall, posing aa risk to the preschool child if it fell. 4. A large wooden toy car in the outdoor play area had worn in spots resulting in exposed sharp edges that could cause injury to a child. Infection Control: 5. The service had a system of storing soothers in individual containers, however inspectors observed on the day that a number of soothers were not in the individual containers and posed a risk of cross contamination. 6. Two mattresses in the baby sleep room were torn with exposed foam. These mattresses are unable to be cleaned effectively and pose an infection control risk. 7. Perishable food items were stored in the children’s lunch boxes on the shelf in the upstairs preschool room. 8. The walls in the downstairs preschool room were flaking making them unable to be cleaned efficiently. Administration of Medication: 9. Medication was stored in an unlocked fridge in the upstairs pre-school room, which was accessible to the preschool children which posed a risk of overdose if ingested. Action submitted by the Registered Provider

- General Safety: 1. The door has been secured. 2. Flexes and chords have been secured 3. A new radiator cover has been bought and secured to the wall. 4. The seating on the toy has been replaced. Infection Control: 5. Staff have been spoken to about using the individual storage containers. 6. Two new mattresses have been purchased. 7. All children’s lunches are now stored in fridge and staff members have been informed of same. 8. Walls have been painted Administration of Medication: 10. Medication will now be stored in a locked box out of reach of children

##### Regulation 25 — First aid

- The first aid boxes available to the preschool children were insufficiently stocked with out of date equipment which could pose a risk to the preschool child

- : In a written response the registered provider stated:
- The first aid boxes have been restocked, and checklists are now in place

##### Regulation 31 — Notification of incidents

- (d) The registered provider failed to notify the agency in writing within three working days of a serious injury to a child that resulted in the child requiring immediate medical treatment by a medical practioner or hospital

- In a written response the registered provider stated. (d) A notification was submitted to the inspectorate after the inspection. These notification will be completed in the time frame required in the future
