#### Inspection of 3 March 2026 — Inspection Report

##### Regulation 16 — Record in relation to pre-school service

- (j) The name of a witness to the administration of medication was omitted on four of the records reviewed

- The person in charge stated that staff have been reminded that a witness signature is required on an administration of medication record. All staff have been made aware of the importance of this

##### Regulation 23 — Safeguarding health, safety and welfare of child

- Infection Control: It was observed that children did not wash their hands after returning inside from outdoor play in the Toddler room. Not ensuring children’s hands are washed after outdoor play posed a risk of infection to the children. Action submitted by the Registered Provider

- Infection control: The person in charge ensured that staff have been reminded of the handwashing policy and the importance of the infection control policy and handwashing after outdoor play. The person in charge sent a policy reminder to all staff

##### Regulation 29 — Premises

- (d) The following were observed as needing repair on day of inspection. 1. Approximately five ceiling tiles in the Toddler room were cracked and broken. Two ceiling tiles in the children’s sanitary adjacent to the Toddler room were stained. One ceiling tile in the Toddler nappy changing room was observed as cracked and broken. These posed a safety risk to children and staff using these rooms. 2. One of the toilets in the Montessori 3 sanitary facility was not maintained in proper working order and required repair. The toilet cubicle door was secured from the interior and an out of order sign was displayed. During subsequent discussion, a staff member advised that the flush mechanism on the toilet was not working, and this was later confirmed when tested by the person in charge

- 1. The person in charge ordered new ceiling tiles to replace the cracked and broken ceiling tiles. The ceiling tiles will be checked regularly going forward to avoid cracks and staining. 2. The person in charge organised for the plumber to come to the service to repair the flush mechanism on one of the toilets. The person in charge stated that all plumbing works will be reviewed regularly

##### Regulation 10 — Policies, procedures etc. of pre-school service

- The staff recruitment policy did not outline how recruitment records would be stored and how long the records would be retained. This posed a potential risk that the appropriate recruitment records may not have been available to review when required

- Person in charge ensured the recruitment policy was updated and contained the relevant and updated information to include retention periods of records. This will be reviewed regularly by management

##### Regulation 16 — Record in relation to pre-school service

- (k) In Pre-Montessori 1 there were 26 incident and accident records reviewed from the 14th of February 2025 and 4 were not signed by a parent or guardian. This poses a potential safety risk that parents or guardians were not made aware that an incident or accident had taken place within the service to ensure adequate follow up care was provided to the child

- The service ensured the four accident and incident forms were signed by parents. Staff were reminded to ensure parents signatures were received and the accident and incident policy was reviewed by staff

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General safety: 1. It was observed on day of inspection that four wooden panels were missing off the steps in the stairway. This posed a risk of tripping or falling to adults and children in the service. 2. There was a tall unstable pile of toys in the hallway adjacent to the sleep room near the Baby room on the upper floor. This posed a risk of injury to children using the hallway as items could have toppled over. 3. There was a high press not securely anchored to the wall in the Baby room. Babies were observed playing alongside this press and pulling themselves up to a standing position using the support of the press. This posed a risk of injury to the babies if the press were to topple over. Infection control: 4. The staff did not support the children to wash their hands prior to dinner time in the Toddler room. This posed a risk of cross infection for the children and was also not in line with the service’s handwashing policy. 5. The changing mat in the nappy changing area located near the toddler room was ripped and therefore could not be effectively cleaned which posed a risk of infection to the children. Action submitted by the Registered Provider

- General safety: 1. The service has ensured the wooden panels would be replaced by the maintenance worker and daily risk assessments would be completed. 2. The pile of toys have been removed and placed into additional storage in the service. Daily risk assessment to include hallways remain clear in the future. 3. The maintenance worker will secure the press to the wall. Staff will ensure to check all high shelving is secured to the wall. Infection control: 4. Staff have reviewed the infection control policy and ensure handwashing is in the daily routine of the service. 5. New changing mats have been purchased, and regular staff checks on the mat will be conducted

#### Inspection of 4 February 2025 — Inspection Report

##### Regulation 23 — Safeguarding health, safety and welfare of child

- General safety: 1.The inspector noted that the water temperature in the sink in the nappy changing area for the Baby room posed a scalding risk to the children as the water temperature was 45°C. Infection control: 2. Children were observed placing used paper towels into open bins in the Toddler and Montessori 1 toilet areas which had no lids and were not pedal operated. This posed a risk of cross infection. 3. Whilst it was acknowledged by management that waterproof mattress covers were available the inspector observed that two mattresses in the baby room did not have a protective cover which made effective cleaning difficult. This was also observed on previous inspection on the 06 March 2024. Action submitted by the Registered Provider The following statement was received from the registered provider:

- General safety: 1. The water temperature has been regulated by manually lowering the boiler and a plumber has been arranged to call to fix the issue. The service plan to monitor the water using a temperature gauge. Infection control: 2. Pedal bins have been purchased and placed in the bathrooms and the nappy changing rooms. The use of these bins is included in the infection control policy. 3. Mattress protectors were available in service for use on these mattresses and further covers were purchased. Staff were reminded about the importance of using these
